摘    要:目的:比较动力加压钢板(dynamic compression plate,DCP)和锁定加压钢板(1ocking compres-sion plate,LCP)治疗肱骨干骨折的效果。方法:对2007年6月~2010年6月于外侧入路下行DCP、LCP分别治疗18例、11例肱骨干骨折的临床资料进行回顾性分析。结果:术后均随访5~20个月,平均10.5个月。DCP组骨折于14~30周愈合,平均16.7周,包括1例固定失败行翻修手术者。LCP组骨折于13~19周愈合,平均13.8周。LCP组较DCP组骨折平均愈合时间明显缩短,两组比较差异有统计学意义(P<0.05)。肩关节功能按Neer评分,DCP组优13例,良5例。LCP组优8例,良3例;肘关节功能按Mayo评分,DCP组优14例,良4例。LCP组优9例,良2例。两组比较差异无统计学意义(P>0.05)。3例术前桡神经损伤者于术后14~28周基本恢复,平均18.3周。两组均无术中神经损伤。结论:切开复位结合LCP内固定是治疗肱骨干骨折安全有效的方法。

Abstract:Objective: To compare the effects of dynamic compression plate(DCP) and locking compression plate(LCP) on humeral shaft fracture.Methods: Data of 29 patients with humeral shaft fracture were treated with the DCP(18 cases) or LCP(11 cases) from Jun 2007 to Jun 2010 were retrospectively analyzed.Results: An average of 10.5 months follow up ranging from 5 to 20 months was conducted in these patients.The union of fractures was achieved in an average of 16.7 weeks(ranging from 14-30 weeks) for those underwent DCP which was significant shorter than the corresponding 13.6 weeks(ranging from 13-19 weeks) for patients had LCP(P〈0.05).According to the Neer Scoring System for the shoulder function evaluation,13 patients had an excellent result,5 had a good result in DCP group,the corresponding number was 8 and 3 in LCP group.The damaged function of the radial nerves in 3 cases mostly recovered in an average of 18.3 weeks ranging from 14-28 weeks.Conclusion: Open-reduction skill combing with LCP fixation are effective and safe treatment the humeral shaft fracture.
