动脉内溶栓治疗急性缺血性脑梗死

摘    要:目的探讨动脉内溶栓治疗急性缺血性脑梗死的临床疗效。方法对7例发病在8h以内的急性缺血性脑梗死患者进行动脉内溶栓治疗。尿激酶平均用量50万u。病人在入院时由神经科医生以美国国立卫生研究院卒中分数(NIHSS)综合评分。临床结果评价在溶栓3个月后进行,按改良的(MRS)分为好结果(MRS0~3)、坏结果(MRS4~6)两类。结果本组患者溶栓治疗后复查脑血管造影,其中4例完全再通,3例部分再通,6例患者为好结果,1例患者为坏结果。结论动脉内溶栓治疗急性缺血性脑梗死是一种安全有效的方法,可大大提高临床治疗效果。

Abstract:Objective To evaluate the clinical effects of intra-artery thrombolysis therapy for acute ischemic cerebral infarction (AICI). Methods 7 patients of AICI in 8 hours were treated with intra-arterial thrombolysis using urokinase of median dose 5 million IU. They were assessed by neurologists using National Institutes of Health Stroke Scale (NIHSS) score at admittance to the hospital. Clinical outcome was assessed after 3 months and classified as good for modified rankin scale (MRS) scores of 0 to 3 and poor for 4 0.6. Results 4 patients received complete recanalization of this group and 3 patient partial recanalization as follow-up. 6 cases had good outcome and 1 poor. Conclusion Intra-artery thrombolysis therapy for AICI might be safe and effective. It could increase as higher clinical treatment effects.
