中晚期胆囊癌手术方式的临床分析

目的：探讨中晚期胆囊癌手术方式与预后关系。方法：回顾性分析1997年1月至2005年1月间收治的85例中晚期胆囊癌的临床资料，施行胆囊癌根治性切除术40例，扩大根治术25例，姑息性手术20例。结果：均获得病理诊断，腺癌51例（60％），最为常见。65例行根治术＋扩大根治术和85例总的术后的1年、3年、5年生存率分别为80．0％、61．5％、49．2％；67．1％、47．1％、37．6％。结论：胆囊癌根治术＋扩大根治术是提高中晚期胆囊癌切除率和疗效的有效方法。

Clinical analysis of operative manner on gallbladder carcinoma.

Objective:To investigate the relationship between operation manner on medial and later gallbladder carcinoma and prognosis.Methods:Retrospective analysis of clinical data of 85 paitents with medial and later gallbladder carcinoma was perfirned,40 patients were performed with gallbladder cancer radical cholecystectomy,extended radical resection was undergone on 25cases,palliatove operation was used for 20 cases.Results:All of patients was diagnosed by pathologic examination,51 patients(60%) were adenocarcinoma,65 cases with radical cholecystectomy or extended cholecystectomy and of 85 patients 1,3,5 year survival rates were 80.0%,61.5%,49.2% and 67.1%,47.1%,37.6%.Conclusion:Radical cholecystectomy or extended cholecystecomy is an active and efficient management for medial and later stage of gallbladder cancer.
