Expression of serum GM in AIDS complicated with Talaro-mycemarneffei's bacteremia and its early diagnosis value

Objective To observe the expression of serum galactomannan(GM)in patients with acquired immunodeficiency syndrome(AIDS)complicated with Talaro-mycemarneffei(TM)bacteremia,and to investigate its early diagnosis value.Methods A total of 247 patients with AIDS complicated with TM from June 2018 to June 2019 were divided into TM infection group,aspergillus infection group,other invasive fungal infection(IFI)group,non-IFI group.The level of serum GM in each group was measured by competitive ELISA,and the diagnostic indexes were calculated and evaluated.Results The level of GM in TM infection group was(0.80±0.77)μg/L,which was lower than that in aspergillus infection group and higher than that in other IFI group and non IFI group,and the differences were statistically significant(P<0.05).The difference the level of GM between other IFI group and non-IFI group was not statistically significant(P>0.05).When the critical values of GM were 0.65,0.75 and 0.85μg/L,the sensitivity of GM test in TM infection group was 35.63%,29.55%and 26.31%,respectively,which was lower than that in the aspergillus infection group,but higher than that in other IFI group and non-IFI group,and the differences were statistically significant(P<0.05).The difference between other IFI group and non-IFI group was not statistically significant(P>0.05).Conclusion The serum GM test can be used as an early diagnostic index of AIDS complicated with TM bacteremia,but the sensitivity is low and there is a false positive risk.It should be combined with other indicators for rapid diagnosis.
