血小板计数与血栓弹力图参数相关性研究

目的探讨血小板(PLT)计数与血栓弹力图(TEG)参数之间的相关性,为临床诊疗提供相关信息。方法对2018年9月至2019年7月绵阳市第三人民医院收治的11535例患者的相关检测指标进行回顾性分析。结果弹力图最大切角(Angle)、血凝块最大振幅(MA)、凝血综合指数(CI)与PLT计数呈正相关,相关系数r分别为0.362、0.463、0.312(P<0.001);反应时间(R)、血凝块形成时间(K)与PLT计数呈负相关,相关系数r分别为-0.022、-0.228(P<0.05)。PLT轻度降低组(PLT计数为75×10^9/L~<100×10^9/L)的PLT计数与MA及CI呈正相关,PLT轻度升高组(PLT计数为300×10^9/L~<450×10^9/L)、PLT正常低值组(PLT计数100×10^9/L~<200×10^9/L)、PLT正常高值组(PLT计数为200×10^9/L~<300×10^9/L)PLT计数与Angle、MA及CI呈正相关(P<0.05)、与K呈负相关(P<0.05),其余各组PLT计数与TEG参数之间均无相关性(P>0.05)。结论PLT计数与K和R呈负相关;与Angle、MA及CI呈显著正相关,PLT计数异常时,MA比PLT计数更能准确地反映PLT功能。

Correlation between platelet count and thromboelastography parameters

Objective To explore the correlation between platelet(PLT)count and thromboelastography(TEG)parameters,and to provide relevant information for clinical diagnosis and treatment.Methods From September 2018 to July 2019,the relerant dection indexes of 11535 patients in the hospital were analyzed retrospectively.Results There was a positive correlation between the maximum tangent angle(Angle),the maximum amplitude of clot(MA),and the composite index of clot(CI)and PLT count(r=0.362,0.463,0.312,P<0.001).And reaction time(R)and clot formation time(K)had the negative correlations with PLT count(r=-0.022,-0.228,P<0.05).PLT mild reduction group(PLT count was 75×10^9/L-<100×10^9/L)PLT count was positively correlated with MA and CI,and PLT count of PLT slightly elevated group(PLT count was 300×10^9/L-<450×10^9/L),PLT normal-low value group(PLT count was 100×10^9/L-<200×10^9/L),PLT normal-high value group(PLT count was 200×10^9/L-<300×10^9/L)were positively correlated with Angle,MA and CI(P<0.05),while negatively correlated with K(P<0.05).There was no correlation between the TEG parameters of the remaining PLT counts(P>0.05).Conclusion PLT count is negatively correlated with K and R,and it is positively correlated with Angle,MA and CI.When PLT count is abnormal,MA could more accurately reflect PLT function than PLT count.
