摘    要:目的分析急性带状疱疹(AHZ)患者外周血中调节性T细胞(Treg)和白细胞介素(IL)-35水平与带状疱疹后遗神经痛(PHN)之间的关系。方法选取2017年11月至2018年10月在福建医科大学附属闽东医院就诊的68例AHZ患者作为观察组,并选择同期20例体检健康者作为对照组。3个月后随访,根据是否出现PHN将患者分为PHN组和无PHN组。检测各组外周血CD4+T细胞和Treg的水平,单个核细胞中IL-35EB病毒诱导基因3(EBI3)和P35]mRNA的相对表达水平,以及血清IL-35水平,分析上述指标在不同组别中的差异。结果与对照组比较,观察组CD4+T细胞水平下降,Treg水平升高,差异均有统计学意义(P<0.001)。PHN组的CD4+T细胞水平低于无PHN组,而Treg水平高于无PHN组,差异均有统计学意义(P<0.05)。与对照组相比,观察组EBI3 mRNA的相对表达水平降低(P<0.05),而P35 mRNA相对表达水平2组差异无统计学意义(P=0.096)。PHN组与无PHN组IL-35(EBI3和P35)mRNA相对表达水平差异无统计学意义(P>0.05)。观察组血清IL-35水平低于对照组(P<0.01),而PHN组与无PHN组IL-35水平差异无统计学意义(P>0.05)。结论在急性带状疱疹中Treg参与负向调节T细胞亚群,与PNH的发生有关;而IL-35可能不参与Treg的细胞免疫抑制过程。

Abstract:Objective To analyze the relationship between the levels of regulatory T cells(Treg)and interleukin-35(IL-35)in the peripheral blood of patients with acute herpes zoster(AHZ)and postherpetic neuralgia(PHN).Methods A total of 68 patients with AHZ who were treated in Mindong Hospital Affiliated to Fujian Medical University from November 2017 to October 2018 were selected as the observation group,and 20 healthy people with physical examination during the same period were selected as the control group.Three months later,the patients were followed up and divided into PHN group and non-PHN group according to whether PHN appeared or not.The levels of CD4+.T cells and Treg in peripheral blood,the relative expression levels of interleukin(IL)-35Epstein Barr virus-induced gene 3(EBI3)and P35]mRNA in mononuclear cells,and the level of serum IL-35 were detected,the difference of the above indicators in different groups were analyzed.Results Compared with the control group,the level of CD4+T cells decreased and the level of Treg increased in the observation group(P<0.001).The level of CD4+T cells in the PHN group was lower than that of the non-PHN group,while the level of Treg was higher than that of the non-PHN group(P<0.05).Compared with the control group,the relative expression level of EBI3 mRNA in the observation group was decreased(P<0.05),but there was no statistically significant difference in the relative expression level of P35 mRNA(P=0.096).There was no statistically significant difference in the relative expression level of IL-35(EBI3 and P35)mRNA between PHN group and non PHN group(P>0.05).The level of serum IL-35 in the observation group was lower than that in the control group(P<0.01),but there was no significant difference in IL-35 level between the PHN group and the non-PHN group(P>0.05).Conclusion In acute herpes zoster,Treg participate in the negative regulation of T cell subsets,which is related to the occurrence of postherpetic neuralgia,while IL-35 may not participate in the cellular immunosuppressive course of Treg.
