HB&L微生物培养仪在尿路感染中的临床诊断价值

目的探讨HB&L微生物培养仪对尿路感染(UTI)的诊断价值。方法收集2019年6-12月在该院就诊的疑似UTI的277例患者作为研究对象,采集患者尿液标本,分别采用尿培养及HB&L微生物培养仪判断患者UTI情况,采用质谱仪鉴定阳性患者菌种分布情况,分析HB&L微生物培养仪培养对UTI的诊断价值。结果HB&L微生物培养仪培养时间与尿培养时间比较,差异有统计学意义(P<0.05);277例尿液标本中,尿培养阳性检出率(23.47%)与HB&L微生物培养仪阳性检出率(23.10%)比较,差异无统计学意义(χ^2=0.010,P>0.05);以尿培养结果作为金标准,HB&L微生物培养仪的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为96.92%、99.53%、99.28%、98.44%、99.06%;单一菌感染55例,混合菌感染10例,采用HB&L微生物培养仪鉴定菌种检出率为90.67%,错误率为1.33%,未检出率为8.00%。结论与尿培养比较,HB&L微生物培养仪能够有效缩短培养时间,且二者阳性检出率比较无差异,HB&L微生物培养仪鉴定菌种的检出率较高。

Clinical diagnostic value of HB&L microbial culture apparatus in urinary tract infection

Objective To investigate the value of HB&L microbial culture apparatus in the diagnosis of urinary tract infection(UTI).Methods A total of 277 patients who suspected UTI in the hospital from June 2019 to December 2019 were selected as research objects,urine samples from patient were collected,UTI of patients were determined by urine culture and HB&L microbial culture apparatus,the distribution of strains in positive patients was identified by mass spectrometry,the diagnostic value of HB&L microbial culture apparatus for UTI was analyzed.Results There was significant difference between the culture time of HB&L microbial culture apparatus and urine culture(P<0.05).There was no significant difference between the positive rate of urine culture(23.47%)and HB&L microbial culture apparatus(23.10%,χ^2=0.010,P>0.05).When the results of urine culture were taken as the gold standard,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 96.92%,99.53%,99.28%,98.44%and 99.06%,respectively.There were 55 cases of single bacteria infection and 10 cases of mixed bacteria infection.The detection rate of bacteria identified by HB&L microbial culture apparatus was 90.67%,the error rate was 1.33%,and the undetected rate was 8.00%.Conclusion Compared with urine culture,the HB&L microbial culture apparatus has shorter incubation time,and there is no significant difference in the positive rate between urine culture and HB&L microbial culture apparatu.The detection rate of strains by HB&L microbial culture apparatus is high.
