无创正压通气联合纳洛酮治疗COPD伴急性呼吸衰竭的疗效观察

目的:探讨无创正压通气(NIPPV)治疗慢性阻塞性肺疾病(COPD)伴急性呼吸衰竭时.采用纳洛酮给予辅助治疗的临床效果。方法:将本院近3年以来收治的84例COPD急性发作伴急性呼吸衰竭患者随机分成两组,分别是对照组41例、治疗组43例。其中,对照组给予抗炎抗感染、解痉平喘、祛痰、肾上腺皮质激素.纠正电解质紊乱以及维持酸碱平衡等常规治疗。在常规治疗的基础上,治疗组则采用无创正压通气联合纳洛酮(1.2～2.0mg/d静脉注射)治疗。对照组与治疗组均以7d为1个疗程,比较两组的临床疗效、气血变化以及患者的苏醒时间。结果:对照组的临床有效率为83%,治疗组的临床有效率为93%,治疗组的疗效明显高于对照组(p<0.05)。另外,治疗组患者的血气指标值与苏醒时间均优于对照组。结论:无创正压通气联合纳洛酮治疗COPD伴急性呼吸衰竭的临床效果显著。

Curative Effect Observation of Noninvasive Positive Pressure Ventilation Combined with Naloxone Treatment of COPD with Acute Respiratory Failure

Objective:Observing the curative about COPD with acute respiratory failure therapy with noninvasive positive pressure ventilation combined with naloxone.Methods:Will be admitted to hospital the past 3 years.84 patients with COPD with acute respiratory failure,acute hair companion were randomly divided into two groups,namely the control group of 41 patients,the treatment group of 43 cases.One control group was given anti-inflammatory,antispasmodic antiasthma tic.expectorant,adrenocorticotropic hormone,to correct electrolyte imbalance and to maintain acid-base balance and other conventional treatment.On the basis of conventional therapy,the treatment group using non-invasive positive pressure ventilation combined with naloxone(1.2～2.0mg / d intravenous) treatment.Control group and treatment group are 7d as a course of treatment,the clinical efficacy of the two groups,blood changes,and recovery time of patients.Results:The clinical control group was 83%effective,the treatment group,clinical response rate was 93%,the efficacy of the treatment group was significantly higher(p<0.05).In addition,patients treated with blood gas values and recovery time than the control group.Conclusion:There is a significant clinical effect of COPD with acute respiratory failure therapy with noninvasive positive pressure ventilation combined with naloxone.
