慢性阻塞性肺病急性加重期机械通气患者60例早期肠内营养支持疗效分析

摘    要:目的:探讨早期肠内营养支持治疗慢性阻塞性肺疾病急性加重机械通气患者的效果。方法:选自我院2008年1月~2010年10月收治的60例慢性阻塞性肺疾病急性加重机械通气患者,随机分成对照组和观察组,两组均在常规抗感染,解痉,机械通气,纠正水电解质、酸碱平衡紊乱,祛痰等治疗的基础上,对照组给予肠外营养,观察组给予肠内营养,比较两组营养效果及不良反应发生情况。结果:观察组治疗后血清白蛋白、前白蛋白、胆固醇与对照组相比,差异有统计学意义(P<0.05),观察组血红蛋白略高于对照组,差异无统计学意义(P>0.05)。观察组营养状况明显优于对照组。两组治疗过程中均未见明显不良反应发生。结论:经过一段时间的治疗发现,肠内营养方法不仅提供人体全部的营养素,补充自然饮食及肠外营养支持的不足,还能有效地维持肠道黏膜的完整性,可提高免疫力,安全性较高。值得临床推广应用。

Abstract:Objecttve:To explore the application effect of early enteral nutrition in the treatment of patients with acute exacerbated chronic obstructive pulmonary disease by applying mechanical ventilation.Methods:60 patients with mechanical ventilation were randomly divided into two groups,observation group and control group,afford anti-infcction,spasmolysis,mechanical ventilation,rectification water and electrolyte acid base equilibrium,eliminating phlegm,control group use parenteral nutrition.observation group use enteral nutrition. Compare the nutrition effect and occurrences of adverse reaction.Results:There has statistic significance in serum total protein,serum albumin,nitrogen balance in patients of the observation group elevated obviously higher than those of the PN group at same stage.There has no statistic significance in hemoglobin between two group.The two groups have no obviously adverse reaction.Conclusion:Early enteral nutrition can improve the nutritional status and prognosis of patients with mechanical ventilation.It is worth to apply in clinical.
