可弯曲喉罩在鼻内镜手术应用的优势探讨

目的:探讨与气管插管相比,可弯曲喉罩(Flexible Laryngeal Mark Airway)在全麻鼻内镜手术(Endoscop Sinus Sinus Surgery,ESS)中的应用优势。方法:择期行ESS手术成年患者60例,随机分为2组(F组和T组),每组30例。观察两组的血流动力学情况、术后恢复情况、手术出血量比较、用药情况、需用盐酸乌拉地尔和盐酸艾司洛尔控制性降压的例数等。结果:F组血流动力学稳定性明显优于T组,P均<0.01;术后恢复情况:F组恢复时间均短于T组,P<0.05;F组恢复质量优于T组,P均<0.05;F组手术出血量低于T组,P<0.05;用药量:丙泊酚和七氟烷用量差别无统计学意义,P>0.05,F组瑞芬太尼用量少于T组,P<0.05;术中需降压例数F组少于T组,P<0.05。结论:可弯曲喉罩与气管插管相比在ESS术中的应用更具优势。

The Application Advantages of Flexible Laryngeal Mask Airway In EndoscopicSinus Surgery

Objective:Compared with endotracheal tube,discuss the Application advantages of Flexible Laryngeal Mask Airway in Endoscopic Sinus Surgery(ESS).Methods:Choose sixty ASAⅠorⅡselected ESS adult patients.Randomly divide them into two groups,F-team and T-team.Each group composes of 30 patients.Observation items conclude hemodynamic conditions,the postoperative recovery conditions, quantity of bleeding during the surgery,the exhausted dosage of propofol,remifentanil and sevoflurane and the controlled hypotension cases by urapidil hydrochloride and esmolo hydrochloride between the two teams.Results:The hemodynamics of the F-team is more stable than the T-team, P<0.01;the recovery time of the F-team is obviously less than the T-team,P<0.01.the recovery quality of the F-team is much better than the T-team, P<0.05;the quantity of bleeding during the surgery of the F-team is lower than the T-team,P<0.05;the dosage of remifentanil of the F-team is lower than the T-team P<0.05;the controlled hypotension cases of the F-team is less man the T-team,P<0.05.Conclusion:The flexible laryngeal mask airway is more applicable to ESS.
