右肺中叶病变的诊断及外科治疗

目的探讨右肺中叶病变的诊断和外科治疗适应症。方法 2002年3月～2009年6月期间在我科行手术治疗的33名右肺中叶病变患者进行分析总结。结果 33名患者中,恶性病变21例,良性病变12例,恶性病变率为63.6%（21/33）,其中40岁以上患者恶性比率高达71.4%（20/28）。全组患者中行中叶楔形切除者3例,中叶切除者19例,行中、下或者中、上两叶切除者9例,行中叶袖状切除1例,右侧全肺切除者1例,无手术死亡患者,术后3例出现并发症,1例肺不张、2例少量胸腔积液。结论右肺中叶病变,有手术指征者应积极行外科手术治疗,且采用外科治疗安全可靠。

Diagnosis and surgical treatment of pathologic changes in middle lobe of right lung

Objective To investigate the indication of diagnosis and surgical treatment of pathologic changes in middle lobe of right lung.Methods 33 cases with pathologic changes in middle lobe of right lung who underwent surgical treatment during March,2002 to June,2008 were studied and analyzed.Results Of the reviewed 33 patients,there were 12 benign cases（63.6%）and 21 malignant cases（36.4%）,the malignant rate of the cases over 40 years old was 71.4%.3 cases underwent wedge resection,19 patients underwent middle lobectomy,bi-lobectomy with either upper or lower lobe was performed in 9 cases,sleeve lobectomy was performed in 1 case and right pneumonectomy was performed in 1 case.Conclusion For those patients with pathologic changes in middle lobe of right lung,surgical treatment is safe and reliable and should be performed.
