经输尿管镜钬激光碎石治疗复杂性输尿管结石

目的探讨复杂性输尿管结石经钬激光腔内治疗的有效性及安全性。方法回顾性分析2006年12月～2007年12月收治的经输尿管镜钬激光治疗复杂性输尿管结石47例的临床资料。47例均伴有患侧肾盂积水，其中8例合并结石远端输尿管狭窄，27例合并有息肉或肉芽组织包裹，12例为ESWL治疗失败后。结果远段结石36例均一次完全粉碎，2周内碎石排尽，成功率100％。近段结石11例，9例一次粉碎，2周内碎石排尽，成功率82％。无严重并发症。30例随访3～8个月，肾积水明显减轻，未发现输尿管再狭窄。结论输尿管镜下钬激光碎石术治疗复杂性输尿管结石安全、有效。

Transureteroscopic holmium:YAG laser lithotripsy for the treatment of ureteral complicated calculi

Objective To evaluate the therapeutic effect and safety of Holmium: YAG laser on the endos copic treatment of ureteral complicated calculi. Methods Investigated 47 cases clinical treatment with ureteral complicated calculi on file between December 2006 to December 2007. The clinical material on Holmium: YAG laser endoscopic treatment of all 47 cases have hydronephrosis. Among them, 8 cases with ureteral stricture, 22 cases with polypus and 12 cases fail of ESWL.Results All of 36cases in the distal ureter were completely fragmented with success rate 100%; 9 of 11 cases in the proximal ureter were fragmented with success rate of 82%. No severity complications occurred. 23 cases were follow-up for 3-8 mouths with significant relief of hydronephrosis and no ureteral restenosis.Conclusion Treatment of urinary calculi with this method has been safe, effective and less invasive.
