青光眼术后浅前房的原因及处理

目的　探讨青光眼术后浅前房形成的原因和处理方法。方法　分析青光眼患者 14 7例 182只眼术后发生浅前房的原因和处理方法。结果　 14 7例 182只眼术后发生浅前房 62例 70只眼 ( 3 8.4% ) ,时间在术后 1～ 14天 ,其中滤过过畅为 47只眼 ,脉络膜脱离 19只眼 ,结膜漏 2只眼 ,恶性青光眼 2只眼。手术恢复前房 6只眼 ( 8.5 % ) ,其余均非手术治疗恢复前房 64只眼 ( 91.5 % )。结论　青光眼术后浅前房发生最常见的原因为房水滤过过畅 ,通常发生在术后 1～ 2天。大多数浅前房可通过非手术治疗恢复 ,必要时采取手术治疗。青光眼术后应密切眼压及眼底情况。

The Causes and Treatment of Shallow Anterior Chamber After Glaucoma Surgery

Objective To investigate the causes and treatment of shallow anterior chamber after glaucoma surgery. Methods The causes and treatment of shallow anterior chamber in 182 eyes of 147 glaucoma patients after glaucoma surgery in our hospital from 1997 to 2002 were analyzed retrospectively. Results The incidence of shallow anterior chamber after glaucoma surgery was 38.4%. Shallow anterior chamber appeared at 1 to 14 days after surgery. Main cause was excessive aqueous humor runoff (47 cases). The others were detachment of choroids (19 cases), conjunctiva leak (2 cases), malignant glaucoma (2 cases). Most of them were treated with non-surgical therapy, and six cases received surgical therapy. Conclusions The common cause of shallow anterior chamber after glaucoma surgery was excessive aqueous humor runoff; it often appeared at one to two days after surgery. It may be managed with non-surgical therapy usually, and surgical interference should be taken if necessary. Intraocular pressure and fundus of eye should be closely monitored after surgery.
