骨泥的制备及其诱导成骨实验研究

摘    要:目的研制一种可随意塑型、填充不规则骨缺损的骨植入材料并观察其成骨效果。方法以羧甲基纤维素钠(CMCNa)为粘合剂复合脱矿骨粉制备骨泥,并将骨泥植入大鼠股肌袋内,通过大体观察,X线片,碱性磷酸酶(ALP)测定,组织学观察及成骨面积测定,研究该植入物在肌袋内的成骨效果。结果CMCNa植入体内4周完全被降解吸收,无毒性反应。与骨粉粘合性好,可随意塑型,对骨缺损填充完全。动物实验大体观察,伤口无红肿及脓性渗出,愈合良好,无死亡。X线片显示骨泥不离散。全脱矿骨泥植入体内后2周,ALP均值高于表面脱矿骨泥组。表明全脱矿骨泥植入后可形成软骨和骨组织,成骨良好。结论CMCNa复合全脱矿骨粉制备的骨泥具有诱导成骨活性,可随意塑型,使用方便,可为临床提供一种填充不规则骨缺损的骨植入材料。

Abstract:Objective To develop a kind of embedding material which can be shaped to heal irregular bone defect and observe its osteogenic capability.Methods Bone cement is made of compound demineralized bone powder,with sodium carboxymethyl cellulose(CMCNa)as adhesive.Then the bone cement was embedded in vastus sack of rats.And osteogenic effect was detected by macrography,X-ray,ALP determination and historical oberservation and osteogenic area oberservation.Results Four weeks after implantaion,CMCNa was completely degraded and absorbed.No toxic reaction was found and CMCNa cohered with bone powder well.And bone cement can be shaped freely and heal the bone defect completely.There was no toxic reaction.The wound was recovered well and there was no swelling and purulent effusion.No animal death was found.The Xray showed the bone cement remained in the location and did not disperse.Two weeks later,the value of ALP in rats with complete dismineralized bone cement implantation was significantly higher than that in the group with superficial enbedded dismineralized bone cement at 2 weeks,which indicated that complete dismineralized bone cement formed cartilage and bone tissue.But there was no significant difference at 4 weeks.Group A and group C had cartilage and bone formation in histology,while group B had no significant bone formation.Conclusion CMCNa compound demineralized bone powder has beneficial osteoinductive and can be easily shaped.It can be used in clinic as embedding material to fill irregular bone defects.
