成人肠套叠的外科诊治分析

目的探讨成人肠套叠的发病机制和外科诊治方法。方法回顾性分析32例成人肠套叠患者的临床表现、类型、诊断及治疗方法。结果阵发性腹痛32例，恶心呕吐23例，腹部包块15例。引起肠套叠原因主要是炎性水肿（4例）、良性肿瘤（10例）和恶性肿瘤（14例）。32例均接受手术治疗，30例恢复良好，2例死于肠坏死引起的感染中毒性休克。结论成人肠套叠多有明确的病理因素，主要为良、恶性肿瘤。超声对确诊该病有一定价值，CT 是首选的检查方法。成人肠套叠一经确诊首选手术探查，手术方式的选择根据肠套叠的部位和类型、原发病的性质等而定。

Analysis on surgical treatment of adult intussusception

Objective To investigate the pathogenesis and surgical treatment method of intussusception in adults. Methods The manifestation,type,diagnosis and treatment of 32 adult patients with intussusception were analyzed retrospectively. Results Thirty-two pa-tients had abdominal pain,23 patients had nausea and vomiting and 15 patients had palpable abdominal mass. The main causes of intussus-ception were inflammation edema(4 patients)and benign(10 patients)or malignant tumor(15 patients). All 32 patients were operated,of whom 30 cases had excellent recovery and 2 cases died of infection shock due to intestinal necrosis. Conclusion Adult intussusception has much clear pathological factors,which are mainly the benign and malignant tumors. Ultrasound has a certain value in confirming the disease, and CT is the preferred method of inspection. The selection of the best operation method in treating adult intussusception should be mainly based on the part of the small intestine,types and nature of the primary disease.
