超声骨刀与横T形截冠法在下颌水平阻生智齿拔除术中的效果比较

目的 比较超声骨刀与横T形截冠法在下颌水平阻生智齿拔除的应用效果。方法 选取2018年1月—2021年12月赤峰学院附属医院行下颌水平阻生智齿拔除的122例患者作为研究对象。将患者随机分为研究组和对照组，每组61例。对照组采用超声骨刀法拔除患牙，研究组采用横T形截冠法拔除患牙。统计两组患者手术时间、术中出血量，记录两组术中焦虑程度，对比两组术后疼痛情况，统计两组患者手术并发症发生情况。结果 两组患者手术时间、术中出血量比较，差异无统计学意义（P>0.05）。研究组术中焦虑程度低于对照组（P <0.05）。两组术后1 d疼痛程度比较，差异无统计学意义（P>0.05）。两组患者术前与术后3 d PEG2、SP、5-TH的差值比较，差异无统计学意义（P>0.05）。两组患者术后并发症总发生率比较，差异无统计学意义（P>0.05）。结论 超声骨刀与横T形截冠法用于下颌水平阻生智齿拔除均可获得满意手术效果，两种方案手术效果类似，但横T形截冠法更有助于缓解患者术中焦虑。

Comparison of the effect of piezosurgery and T-shaped splitting in the extraction of impacted mandibular molars

Objective To compare the effect of piezosurgery and T-shaped splitting in the extraction of impacted mandibular molars.Methods A total of 122 patients who underwent extraction of impacted mandibular molars in Affiliated Hospital of Chifeng University from January 2018 to December 2021 were selected and divided into research group and control group, with 61 patients in each group. The control group was treated with piezosurgery, and the study group was treated with T-shaped splitting to extract the affected teeth. The operative duration and intraoperative blood loss of the two groups were recorded, and the anxiety in the two groups was observed. The postoperative pain was compared between the two groups, and the incidence of surgery-related complications in the two groups was analyzed.Results There was no significant difference in the operative duration and intraoperative blood loss between the two groups (P > 0.05). The anxiety in the study group was lower than that in the control group (P < 0.05). There was no significant difference in the pain between the two groups of patients on the first day after the operation (P > 0.05). There was no statistical significance in the differences of prostaglandin E2 (PGE2), substance P (SP) and 5-hydroxytryptamine (5-HT) before and 3 days after the operation between two groups (P > 0.05). There was no statistically significant difference in overall incidence of postoperative complications between the two groups (P > 0.05).Conclusions Both piezosurgery and T-shaped splitting achieve satisfactory surgical outcomes for the extraction of impacted mandibular molars. Though the surgical efficacy is comparable between the two approaches, T-shaped splitting is more conducive to relieving the anxiety of patients during the operation.
