外伤性后颅窝血肿52例临床分析

目的分析总结外伤性后颅窝血肿的诊断和治疗。方法对2000年1月至2005年12月共52例外伤性后颅窝血肿的影像:诊断和治疗进行分析。结果本组52例外伤性后颅窝血肿中硬膜外血肿39例(75%),硬膜下血肿2例(3.85%)小脑血肿11例(21.15%)。手术42例,保守治疗10例。伤后半年行GOS评估:死亡6例,重残1例,预后良好45例。结论后颅窝血肿应及时诊断,根据出血量和GOS评分来选择手术或保守治疗。

Clinical analysis of 52 cases of traumatic hematoma of the posterior fossa

Objective To analyze the diagnostic and therapeutic means of traumatic hematoma of the posterior fossa. Methods The clinical data, ineluding clinical manifestion, radiographic feature and therapeutic methods in 52 cases of traumatic hematoma of the posterior fossa, admitted from January, 2000 to December 2005 were analyzed. Results In this group, epidural hematomas were the most common traumatic mass lession of the posterior fossa, accounting for 39 cases, subdural hematoma were 2, intracerebellar hematoma were 11. 10 cases (hematoma volume<10 ml) were treated with conservative methods, 42 cases were treated by operation. Six months later, Glasgow Outcone Scale (GOS) scores showed that six cases died and one case remained sevene disability, and 45 cases got good recevery. Conclusion The timely diagnosis are essential for the posterior fossa hematoma. the amount of hematoma and GCS index is the key in deciding whether to choose surgical or conservative treatments.
