多层面螺旋CT血管造影在颅内动脉瘤诊治中的初步应用

摘    要:目的:初步评估多层面螺旋CT血管造影(MSCTA)在颅内动脉瘤诊治中的临床应用价值。方法:对57 临床怀疑颅内动脉瘤病例应用Aquilion多层螺旋CT行MSCTA检查,重建方法包括多层面重建(MPR),遮盖容积重建(SVR),最大密度投影(MIP)以及外视镜(Fly-around)和内镜式血管表面重建(Fly-through)。49例确诊为颅内动脉瘤,均经手术证实。还在术后对5例病例进行了MSCTA复查。结果:MSCTA共发现动脉瘤49个,与DSA和手术相比,检出率分别为100%和98%;而且MSCTA能够清楚显示动脉瘤及其与周围结构之间的解剖关系。结论:可将MSCTA作为颅内动脉瘤诊断的首选检查手段。在指导动脉瘤治疗方面MSCTA可对DSA起到重要补充作用,并可在动脉瘤夹闭术后复查中发挥作用。

Abstract:Objective To evaluate the clinical value of multislice helical CT angiography(MSCTA)in the detection and treatment of intracranial aneurysms.Method MSCTA was performed in57patients suspected of harboring intracranial aneurysms.After all data was transferred to a workstation,we used Multiple Plane Rendering(MPR),Shaded Volume Rendering(SVR),Maximum Intensity Projection(MIP),Fly-aroundand Fly-through techniques to reconstruct2D or3D images.59patients with intracranial aneurysms were revealed and confirmed by the following surgery.Postoperative MSCTA was performed in5patients.Result59intracranial aneurysms were detected by MSCTA.Compared with digital subtrac tion angiography(DSA)and operation(including interventional neuroradiological management),the detection rate of MSCTA is100%and98%,respectively.Furthermore,the aneurysms and their surrounding structures were clearly demonstrated.MSCTA provided unique information in management of the intracranial aneurysms.Con clusion MSCTA can be used as the first choice of screening intracranial aneurysms and as an adjunct to DSA in planning of the management for intracranial aneurysms.It is very helpful in postoperative evaluation of the clipped aneurysms.
