全髋关节置换治疗老年股骨颈骨折的疗效分析

目的对全髋关节置换治疗老年股骨颈骨折的疗效进行研究。方法对42例（43髋）65岁以上股骨颈骨折患者行全髋关节置换术后功能恢复情况跟踪随访，平均随访2．8年，随访内容包括术后近、远期并发症，功能评价及影像检查结果。结果43髋中按Harris评分平均为90．2分，优良率占89％（37例），其中28例无疼痛及功能障碍，4例术后出现脑血并发症，1例股骨近端劈裂骨折行翻修。切口均I期愈合。结论全髋关节置换对老年股骨颈骨折是有效的手术方法，严格掌握手术适应证，规范手术操作及术后的后续治疗，对手术成功及防止并发症的发生是至关重要的。

Total hip replacement therapy in elderly femoral neck fracture

Objective To observe the outcomes of total hip replacement therapy in elderly femoral neck fracture. Methods Forty-two cases of 43 hip femoral neck fracture patients who were older than 65-year-old were enrolled. Mean follow-up period was 2.8 years. Long-term complications and functional evaluation of the inspec-tion images were analyzed. Results Fourty-three hips had an average of 90.2 points of Harris score. 28 of them (89%) didnt have pain or dysfunction. After the operation, 4 cases had cerebral vascular complications and 1 case of proximal femoral fracture had a revision. The average time of operation was 1.2-1.5 hours. All cases had a heal-ing of first intension. Conclusion Total hip replacement is an effective therapy for femoral neck fractures in the elderly. It's important to follow strict surgery indications and to apply standard therapies, which is of vital impor-tance for a successful operation and complication-avoidance.
