后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿（附18例报告）

目的 探讨后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的手术方法和临床效果。方法 对我科在2007年7月~2009年12月收治18例乳糜尿患者施行后腹腔镜肾蒂淋巴管结扎进行回顾性分析。结果 18例患者均手术成功,手术时间65～125（95±22）min,术中出血量50～120（75±12）ml,术中术后未发生明显的并发症。16例单侧乳糜尿患者出院时尿液清亮,尿乙醚试验为阴性。随访3～26月无复发。结论 该术式具有微创、出血少、并发症少等特点,淋巴管结扎更加完全,临床效果良好。

Retroperitoneoscopic ligation of renal lymphatic vessels for the treatment of chylu-ria(Report of 18 cases)

Objective To investigate the surgical method and the clinical efficacy of retroperi- toneoscopic ligation of renal pedicle lymphatic vessels for the treatment of chyluria. Methods From July 2007 to December 2009, 18 case with chyluria received retroperitoneoscopic ligation of renal pedicle lymphatic vessels were analyzed retrospectively. Results The average operation time was 65- 125 （95 ＋-22 ） min, the average intraoperative blood loss 50N 120 （75 ＋- 12 ） ml. No complications occurred. The urine of 16 patients was clear when the patients were discharged from the hospital.And Ether test of urine was negative. Among them, the operation time, intraoperative bleedingvolume, intra and postoperative complications and operative effects were observed.No recurrence of ehyluria was found during the follow up time ranged from 3 to 26 months. Conclusions It is an efficient surgical procedure and has the advantages of minimally invasion, less blood loss, less complication and a more complete lymphatic vessels ligation.
