急性脑梗死患者脑脊液兴奋性氨基酸水平的变化及其临床意义

摘    要:目的 研究急性脑梗死患者脑脊液(CSF)兴奋性氨基酸含量变化及其临床意义。方法 采用高效液相层析方法测定30例急性脑梗死患者及20例偏头痛患者(对照组)CSF谷氨酸(Glu)、天门冬氨酸(Asp)的含量。结果 脑梗死组患者CSF中Glu、Asp含量显著高于对照组(均P〈0.001)。脑梗死组患者CSF中Glu和Asp含量与梗死体积呈显著正相关(r=0.75,P〈0.01;r=0.437,P〈0.05),CSF中Clu含星与神经功能缺损程度亦呈正相关(r=0.55,P〈0.01)。结论 脑梗死发生后CSF中Glu、Asp含量明显升高提示其参与了脑梗死的病理生理过程CSP中Glu、Asp含量可反映脑梗死患者的病情及梗死灶的大小。

Abstract:Objective To investigate the changes of excitatory amino acids of cerebrospinal fluid(CSF) in patients with acute cerebral infarction(ACI) and its clinical significance.Methods The CSF levels of excitatory amino acids including glutamate(Glu) and aspartate(Asp) in 30 cases with ACI and 20 control subjects with migraine were measured by high performance liquid chromatography.Results Glu and Asp levels in patients with ACI were significantly higher than those of the controls(all (P<)0.001).Positive correlations between Glu and Asp levels in CSF and infarct sizes were confirmed((r=)0.75,(P<)0.01;(r=)0.437,(P<)0.05).A positive correlation was also found between Glu level and neurological deficit score((r=)0.55,(P<)0.01).Conclusion It is suggested that excitatory amino acids are participated in the pathogenesis of cerebral infarction.The Glu and Asp levels in CSF may be regarded as indexes of the condition of illness and the infarct size.
