羊水污染321例临床分析

摘    要:目的:为探讨分娩期羊水胎粪污染的临床原因,以降低对新生儿的不良影响。方法:选择2006年6月至2007年6月在山西省介休市人民医院分娩的2123例产妇,产程中有羊水胎粪污染≥Ⅱ度的321例产妇,另随机选择同期产程中羊水清亮的330例产妇作为对照组,从诊断、分娩方式、母体并发症及新生儿Apgar评分等方面进行对比分析。结果:2123例产妇中,羊水污染≥Ⅱ度的产妇为321例,发生率为15.12%。母体并发症、胎心监护异常发生率、剖宫产率及新生儿窒息率均高于对照组(P均小于0.01)。结论:分娩期发现羊水污染,应加强胎儿监护,积极处理,宜尽快结束分娩,可降低新生儿的窒息率。

Abstract:Objective:To investigate the clinical causes of meconium-stained amniotic fluid at the stages of labor and reduce the adverse effects for neonates. Methods: A total of 2123 parturients who deliveried in Jiexiu People's Hospital of Shanxi Province from June 2006 to June 2007 were chosen and there were 321 parturients with meconiumstained amniotic fluid (≥Ⅱ degree) in birth process. Another 330 parturients with normal amniotic fluid who were at the same stages of labor were randomly chosen from 2123 cases as a control group and they were carried out the comparative analyses from the diagnosis, ways of labor, maternal complications, the neonate grades ( Apgar ) and other aspects. Results: There were 321 parturients with meconium-stained amniotic fluid ( ≥Ⅱ degree) in 2123 cases and the incidence was 15.12%. The maternal complications, the abnormal incidence of fetal heart electrocardiograph monitoring, cesarean section rate and neonatal asphyxia rate were higher than those of the control group respectively (P 〈 0.01 ). Conclusion: It' s important to strengthen the fetal monitoring, carry out the active treatment and end the labor as soon as possible so as to reduce neonatal asphyxia If the meconium-stained amniotic fluid is found at the stages of labor.
