鼻咽癌单侧咽旁间隙侵犯的放射治疗

摘    要:1988年5月至1989年4月,在我院进行放射治疗并有单侧咽旁间隙侵犯的鼻咽癌60例,随机分成治疗组和对照组。治疗组采用我们自己设计的两侧耳前野、鼻前凸形野和一侧耳后野;对照组采用常规两侧耳前野和鼻前凸形野。放疗结束时,治疗组和对照组的鼻咽原发灶残留率相同,均为13.3%(4/30)。放疗结束后3个月,CT结果显示,治疗组鼻咽原发灶残留率为6.7%(2/30),对照组为10%(3/30),两组差异无显著性(P>0.05);治疗组咽旁间隙残留率为16.7(5/30),对照组为53.3%(16/30),两组差异有非常显著性(P<0.01),提示新的设野方案能明显减少鼻咽癌咽旁间隙侵犯的残留率,取得较好的近期疗效。

Abstract:Sixty NPC patients with unilateral paranasopharyngeal space involement were randomly divided into two groups (treatment group and control group) and treated with radiotherapy from May 1988 to April 1989 in our hospital. The treatment group adopted bilateralpreauricular portals,an anterior nasalumbonate portal and a retroauricular portal which wehad designed. The control group adopted routine bilateral preauricular portals and an anteriornasalumbonate portal. When the radiotherapy ended,the gross residual rate of primary lesionof nasopharynx of both groups was the same, 13. 3% (4/30). Three months after the radiotherapy,CT scan showd that the residual rate of the primary lesion of nasopharynx of thetreatment group was 6. 7% (2/30) and that of the control group 10% (3/30). There was nosignificant difference bwtween the two groups (P>0.05). The residual rate of involvedparanasopharyngeal space of the treatment group was 16. 7% (5/30) and that of the controlgroup 53. 3% (16/30). The difference was statistically significant (PM 0. 01),The resultsshowed that the new method could reduce the residual rate of involved paranasophryngeals pace.
