摘    要:目的现场评价胶体染料试纸条法(DDIA)免疫诊断试剂盒在大山区型血吸虫病流行区筛查病人中的应用价值。方法在云南省大山区型血吸虫病流行区,选择3个自然村作为调查现场, 随机抽取10-70岁的村民作为调查对象,进行血吸虫病病史个案调查;收集粪便标本,采用改良 Kato-Katz法(1送3检)作病原学检查;静脉采血,分离血清,分别采用DDIA诊断试剂盒、酶联免疫吸附试验(ELISA)诊断试剂盒和环卵沉淀试验(COPT)诊断试剂盒进行血吸虫病免疫学诊断。结果 3个村共筛查村民659人,Kato-Katz法粪检阳性187例,阳性率为28.4%;DDIA、ELISA和 COPT的阳性率分别为70.9%,84.2%和54.9%;在187例粪检阳性者中,DDIA、ELISA和COPT 的阳性符合率分别为98.4%、96.8%和81.8%。结论 DDIA免疫诊断试剂盒在大山区型血吸虫病流行区现场大规模筛查病人中具有较高的敏感性和特异性。

Abstract:Objective To evaluate the field application value of the kit of dipstick dye immunoas-say (DDIA) for screening schistosomiasis in mountainous endemic regions. Methods In a mountainous endemic region of schistosomiasis in Yunnan Province, random samples of the villagers aged 10 - 70 years from three natural villages were examined by using the modified Kato-Katz's quantitative thick smear technique for parasitological stool examination, and the sera from the villagers were tested with the kits of DDIA, ELISA and COPT for immune examination respectively. Results A random sample of 659 villagers from three natural villages were screened, the positive rates of the modified Kato-Katz's quantitative thick smear technique, DDIA, ELISA and COPT methods were 28. 4%; 70. 9%, 84. 2% and 54. 9% respectively. The positive coincidence rates of DDIA, ELISA and COPT were 98.4%, 96.8 % and 81.8 % in 187 stool positive subjects. Conclusions The DDIA kit has a high sensitivity and good specificity for screening patients infected with Schistosoma japonicum on large scale in mountainous endemic regions. The assay is fast and easy to be done and available for application in the endemic field.
