腹腔镜胆总管探查术中即时缝合胆总管382例临床研究

目的　探讨运用腹腔镜胆总管探查即时缝合术的治疗经验。方法　回顾性分析我院1 992年 6月至 2 0 0 2年 1 2月运用腹腔镜胆总管探查即时缝合术 382例的诊治经过。结果　手术成功36 4例 ,残石经内镜取石治愈 4例 ,胆漏经腹腔引流管或内镜鼻胆管引流治愈 1 1例 ,细导管脱落 2例 ,晚期胰头癌术后 1 5d死亡 1例。结论　只要选择合适的病例 ,腹腔镜胆总管探查即时缝合术是安全、可行的

Primary closure of bile duct incision in laparoscopic bile duct exploration- A clinical study of 382 cases

Objective To summarize the experience of primary closure of bile duct incision in laparoscopic bile duct exploration for choledocholithiasis and bile duct stenosis.Methods From June 1992 to Dec.2002,clinical data of 382 patients undergoing primary closure of bile duct incision in laparoscopic bile duct exploration were retrospectively analyzed.Results The operation was successful in 364 out of 382 patients.Four patients with residual stones were cured by endoscopic sphincterotomy or papillary balloon dilation.Eleven patients with bile leakage were cured conservatively.Exfoliation of the fine duct occurred in 2 patients.One case with advanced pancreatic head carcinoma died from MSOF 15 days postoperation.Conclusion Primary closure of bile duct incision in laparoscopic bile duct exploration was safe and effective in some patients.
