NIPPV对重症SARS患者的治疗作用及其并发症的临床观察

目的：观察无创正压通气(NIPPV)对重症SAILS患者的疗效及并发症。方法：采用回顾性调查研究的方法，对我院收治的39例重症SAILS患者应用NIPPV治疗时的临床相关资料进行总结和分析。结果：39例应用NIPPV治疗的重症SAILS患者中，显效23例，有效13例，无效3例，总有效率92．3％。因应用NIPPV治疗而出现的并发症共7例，其中皮下气肿伴纵隔气肿3例，皮下气肿伴气胸4例。结论：NIPPV对重症SARS患者的疗效显著；它所引起的并发症可以通过积极的处理得以减少和消除；镇静药物的应用有助于提高NIPPV的疗效，降低有创机械通气的几率。

Clinical observation on non-invasive positive pressure ventilation (NIPPV) and its complications in the treatment of severe acute respiratory syndrome (SARS)

Objective:To observe the role and complications of non-invasive positive pressure ventilation (NIPPV) in the treatment of severe acute respiratory syndrome (SARS). Methods:To summarize and analyze retrospectively the clinical data of NIPPV in the treatment of critical SARS patients. Results:Of the 39 patients treated with NIPPV, 36 patients had effective result. The others had to change the ventilation pattern to invasive positive pressure ventilation (IPPV). The complications with NIPPV occurred in 7 patients, 3 patients had subcutaneous and mediastinal emphysema, 4 patients had subcutaneous emphysema and pneumothorax.Conclusion:NIPPV is an effective method for the treatment of critical SARS, and its complications can be reduced or even eliminated by active management. Satisfied sedation on SARS patients can help to enhance curative effect of NIPPV and reduce the use of IPPV pattern.
