Survivin在乳腺癌组织中的表达及其与临床相关因素的关系

目的探讨Survivin在乳腺癌组织中的表达情况及其与患者预后、生物学指标(原癌基因C-erbB-2、雌激素受体ER)等因素的关系。方法采用免疫组化法——二步法TM对44例术前未行放、化疗的乳腺癌蜡块标本及其癌旁组织进行Survivin、C-erbB-2、ER检测以及对15例正常乳腺组织进行Survivin检测。结果肿瘤组织及癌旁组织中Survivin阳性率分别是55.6%、4.5%。正常乳腺组织中无Survivin表达,三者之间的阳性率有显著性差异(P<0.05),其表达与C-erbB-2呈正相关性,与ER呈负相关性。Survivin表达与患者的3年生存率呈负相关性,表达阴性者生存时间明显高于阳性表达者(P<0.05)。结论Survivin在乳腺癌组织中表达上调,提示该基因在乳腺癌发生发展中起重要作用。Survivin阳性提示预后不良,可作为乳腺癌预后的独立指标。由于Survivin与C-erbB-2和ER对预后的判断相一致,因此,三者联合检测更有助于提高对乳腺癌预后的评价。

Expression of Survivin in Breast Cancer Tissue and Its Relationship with Clinical Factors

Objective To investigate the expression of survivin in breast cancer,and the correlation to the clinical characteristics, prognosis, and the tumor bio-markers. Methods The expression of surviving,c-erbB-2 and ER in the cancer tissue and tumor nests adjacent tissue was investigated by immunohistochemistry in 44 cases of breast cancer without the history of radiotherapy or chemotherapy, and only surviving for normal mammary gland tissue. Results The positive rate of survivin expression 55.6%, 4.5%and 0 in the cancer tissues, tumor nests adjacent tissues and normal mammary gland tissue. There was significantly difference among the three groups. The expression of survivin was positively correlated with c-erbB-2 and negatively with ER and 3-year overall survival rate. The survival time of negative expression of survivin was longer than that of positive expression of survivin (P<0.05). Conclusions There is a high expression level of survivin in breast cancer, it may play an important role in genesis and development of breast cancer. The positive expression of survivin relates with poor prognosis and can be used to evaluate the prognosis as a potential index. The survivin should be considered together with c-erbB-2 and ER for enhancing the accuracy of prognostic evaluation in breast cancer.
