异搏定(Verapamil)对犬出血性休克氧转运的影响

摘    要:采用修改的Wigger法复制出血性休克,使mAP维持5.33kPa(40mmHg)1h。然后夹闭放血管至90min时,将失血在30min内全部输回,再继续观察2h。于失血后60~80min之间,异搏定组(N=6)和对照组(N=7)3次分别静注异搏定0.15mg/kg和等量生理盐水。结果表明,输血后异搏定组动物的总体耗氧量明显增加,与对照组相比输血后1和2h的P值分别小于0.05和0.01;异搏定动物的mAP和左室收缩和舒张压力变化最大速率(±dp/dt max)维持接近失血前水平,输血后2h与对照组相应值相比有明显统计学差异(P值分别小于0.05和0.01);两组动物的心肌耗氧量虽未出现明显差别(P>0.05),但输血2h异搏定组有改善趋势,对照组继续降低。

Abstract:The effects of verapamil on the oxygen transport during a hemorrhagic shock were studied in dogs.The animals were bled from the femoral artery and the mean arterial pressure(mAP)was maintained at 5.33kPa(40 mmHg) for 90 minutes.During the 60 to 80 minutes of hypotension,the animals of the treated group received 3 intravenons injections of verapamil with the dosage of 0.15 mg/kg of body weight,while those of the control received a same amount of normal saline.Then all the blood shed was transfused back to the animals and they were observed for 2 more hours.It was found that the total oxygen consumption of the treated gronp was far much higher than that of the control(P<0.01) The myocardial oxygen consumption,dp/dt max and mAP of the treated group remained constant through the observation period but those of the control were significantly decreased.The difference of dp/dt max and mAP between the 2 groups was significant (P<0.01 for dp/dt max,P<0.05 for mAP).Though the difference of myocardial oxygen consumption was not significant between the 2 groups,but in the treated group,there was a tendency of improvement of myocardial oxygen consumption after the administration of verapamil.Hemorrhagic shock and its resuscitation induces a lowering of plasmaa calcium and an overloading of calcium in the mitochondria to precipitate moycardial damage Verapamil is one of the blocking agents of calcium,and can abolish the overloading of calcium in cells and mitochondria during hemorrhagic shock,which will improve mitochondria respiration and ATP-generation to alleviate cardiac da-mage.
