心力衰竭患者动态血压监测

目的 :探讨心力衰竭患者的昼夜血压变化、左室肥厚及其与心功能受损程度的关系。方法 :6 6例心衰患者按心功能分为 A组 (心功能 级 ) 36例 ,B组 (心功能 级、 级 ) 30例 ,作 2 4 h动态血压监测 (ABPM)、超声心动图 ,并按 Devereux校正公式计算左心室重量指数 (L VMI)。结果 :A、B两组患者中血压昼夜节律减弱或消失分别 5 8.6%、86 .7% (P<0 .0 1) ,B组比 A组有较高的夜间平均收缩压与夜间平均舒张压 ,以及明显增大的 L VMI。结论 :心衰患者血压昼夜节律有明显变化 ,与心功能受损程度相关 ,左室肥厚与心功能受损程度相关。ABPM监测、左室肥厚测定对心衰患者的心功能评价有一定的临床意义。

Ambulatory Blood Pressure Monitoring of Heart Failure Patinets

Objective To explore the relationship between change of ambulatory blood pressure monitoring (ABPM),left ventricular hypertrophy (LVH) and damage of heart function in patients with heart failure.Methods 66 patients with heart failure were divided into two groups: group A (n=36, heart function classⅡ), group B (n=30, heart function class Ⅲ,Ⅳ). The ambulatory blood pressure was measured for 24 hours and the left ventricular mass indexes (LVMI) were measured with echocardiography according to devereux’s correction formulas.Results There is 55.6 % abnormality of ABPM in patients with heart function class Ⅱ, and 86.7 % with class Ⅲ,Ⅳ(P<0.01).nSBP,nDBP and LVMI in group B were significantly higher compared with those in group A respectively (P<0.01).Conclusion The prevalence of ABPM abnormality and LVH were correlated with the degree of heart failure. ABPM and LVH can act as valuable parameters in heart function class.
