UroVysion荧光原位杂交技术在膀胱癌诊断中的应用价值

摘    要:目的 应用UroVysion荧光原位杂交技术检测膀胱尿路上皮癌患者尿液脱落细胞中染色体异常,探讨UroVysion荧光原位杂交技术在膀胱尿路上皮癌诊断中的应用价值.方法 随机留取2010年6月至2012年2月期间107例疑似膀胱尿路移行上皮肿瘤患者的肉眼血尿,行UroVysion荧光原位杂交和尿液脱落细胞学分析,结合活检病理结果,比较UroVysion荧光原位杂交和尿液脱落细胞诊断膀胱癌的效能.结果 通过膀胱镜活检或手术后病理检查明确诊断为膀胱癌患者92例,其中UroVysion荧光原位杂交技术阳性患者70例,其敏感度为76.1%,特异度为92.8%,G1、G2、G3分级敏感性分别为61.2%、81.2%、92.3%,非肌层浸润性肿瘤、肌层浸润性肿瘤的敏感度分别为73.6%及85.0%,尿脱落细胞学总的敏感度为33.7%,特异度为93.3%.结论 UroVysion荧光原位杂交的检测为膀胱尿路上皮癌的早期诊断提供了快速、安全、灵敏度高的无创伤性检测方法.

Research on the application of UroVysion fluorescence in situ hybridization in diagnosis of urinary bladder cancer.

Abstract:Objective To assess the value of UroVysion fluorescence in situ hybridization (FISH) in the diagnosis of bladder cancer. Meth ods Urine samples from 107 patients suspected of having bladder cancer were collected before cystoscopy. These samples were performed simultane ously UroVysion FISH and cytological analysis. The sensitivity and specificity were calculated by software SPSSI3.3 based on the histology and pa thology. Results The overall sensitivity of cytology and FISH was 33.7% and 76.1%. Their specificity was 92.8% and 93.3%. The sensitivity of FISH were 61.2%, 81.2%, 93.3% from low grade tumor to high grade tumor. There was significant difference in total sensitivity for diagnosis and staging of transitional cell carcinoma of bladder between FISH and cytological examination ( P 〈 0. 05 ). Conclusion UroVysion FISH is more sensitive than cytological examination,hence it may be applied as a valuable method for diagnosing transitional cell carcinoma of bladder.
