纳米炭混悬液在示踪cN0喉癌前哨淋巴结中的应用

目的 评价纳米炭混悬液在示踪临床N0期（cN0）喉癌前哨淋巴结中的价值.方法 对20例cN0喉癌患者术中采用纳米炭混悬液进行前哨淋巴结检测,结合术中及术后病理检查结果分析其探测及预测区域淋巴结转移的效果.结果 20例cN0喉癌患者中19例成功显示了前哨淋巴结,发现率为95.0%.每例发现前哨淋巴结1～3枚,平均2.2枚.术中对前哨淋巴结行冰冻病理检查发现3例出现淋巴结转移,和术后HE染色病理检测结果一致.对相应区域淋巴结转移的阳性和阴性预测率均为100%.结论 采用纳米炭混悬液探测及活检前哨淋巴结能够较准确反映cN0喉癌颈淋巴结的转移状态.

Identification of sentinel lymph node with carbon nanoparticles in cN0 laryngeal cancer.

Objective To determine the significance of carbon nanoparticles in tracking the sentinel lymph node （SLN） of cN0 laryngeal cancer. Methods The sentinel lymph nodes of 20 cases of cN0 laryngeal cancer were detected with carbon nanoparticles injection peritumorally. All these sentinel lymph nodes were taken biopsy at the same time. The accuracy in predicting cN0 neck lymph node were analysed. Results In 19 cases （95%） , the sentinel lymph nodes were detected successfully in 95.0% （19/20） of cases. Every case had 1 to 3 SLN, with an average 2.2. Metastasis was determined by intra - operative frozen biopsy, which were in consistence with the post - operative HE patholaogical examina tion. Both the positive and negative prediction rates are 100%. Conclusion It is reliable to predict the neck status of cNO laryngeal cancer with the frozen biopsy of sentinel lymph nodes traced by carbon nanoparticles intra - operatively.
