免疫血红蛋白法与转铁蛋白法检测胃液隐血结果比较

目的 评价并比较免疫血红蛋白法（IFOBT）与转铁蛋白法（Tf）在胃液隐血检测中的应用.方法 收集72例胃液标本（标本需在收集后1 h内检验完成,若不能及时检验需置于4℃冰箱保存）分别用血红蛋白胶体金试纸条、转铁蛋白试纸条进行隐血检测.结果 免疫血红蛋白法共检出阳性标本20例,阳性率为27.78%;转铁蛋白法检出阳性标本41例,阳性率为56.94%,两种检验方法结果差异有显著性（P＜0.05）,转铁蛋白法阳性率高于血红蛋白法.而联合检测（Tf＋IFOBT）阳性率增加到62.5%.通过胃液颜色与隐血检测结果分析发现,当胃液颜色为黑色、黑褐色、咖啡色时,绝大多数隐血试验为阳性.但是其他颜色的胃液也有阳性结果.结论 转铁蛋白法检测胃液隐血优于免疫血红蛋白法,联合检测可提高胃液隐血试验阳性率.

Comparison on results of IFOBT and Tf test in gastric occult blood test.

Objective To evaluate and compare the significance of immuno fecal occult blood test （IFOBT） and transferrin（Tf） test for examination of occult blood in gastric juice. Methods A total of 72 gastric juice samples（ samples should be examined within 1 hour after collec tion,if they can not be completed then they should be stored in 4 ℃refrigerator） were collected for this study, every sample had been tested by both hemoglobin test strip and transferrin test strip. Results IFOBT showed positive result in 20 samples, and the positive rate was 27.78% ;Tf method showed positive result in 41 samples, and the positive rate was 56.94%. There was significant difference between these two methods （ P 〈 0. 005 ）, Tf test had significantly higher positive rate than that of IFOBT. The positive rate of combination of Tf test with IFOBT reached 62.5%. When the color of gastric juice was black, dark brown or brown, most of the occult blood test showed positive result. But other colors also had posi tive results. Technicians should not only distinguish specimens by appearance and color whether they contain blood or not, and they should perform gastric occult blood test for identification. Conclusion The detection of gastric occult blood with transferrin （IF） test is better than IFOBT. Com bination of Tf test with IFOBT can raise the positive rate of gastric occult blood examination.
