隐匿性颈淋巴结转移癌的超声血流特点

胃的研究隐匿性颈淋巴结转移癌的超声血流特点，探讨能量多普勒血流显像（Power Doppler Imaging，PDI）在该病中的诊断价值。方法43例行颈淋巴结清扫术的头颈癌患者，术前接受颈部PDI捡查，选择短轴最大直径〈10mm的淋巴结64含，记录其PDI结果，术后标本经病理检查确定有无癌转移，证实良性淋巴结30个、恶性淋巴结34个，回顾性对比分析良、恶性淋巴结的PDI资料。结果良性淋巴结的血流分布以中央型为主，血流强度多为I级，恶性淋巴结则以周边型或混合型为主，血流强度多为Ⅱ级以上，若以出现周边型或混合型血流作为恶性淋巴结的判定指标，．其诊断敏感性、特异性、准确度分别为76．5％、93．3％、84．4％，血流速度、血管阻力指数、搏动指数在良、恶性淋巴结间无显著性差异。结论隐匿性颈淋巴结转移癌多表现为淋巴结周边出现丰富血流，PDI对其具有重要的诊断价值。

Blood flow features of occult cervical lymph nodes metastases of cancer with color doppler imaging

Objective To investigate the blood flow feature of the occult cervical lymph nodes metastases of cancer and explore the diagnostic value of power Doppler Imaging（PDI）. Methods The PDI results of 64 pathologically verified cervical nodes（ 30 benign, 34 malignant）obtained from 43 cases of head and neck cancer and measured less than 10mm in minimal axial diameter were analyzed retrospectively. Results Benign cervical lymph nodes showed characteristics of central vascular pattern and grade I vaseularity, malignant cervical lymph nodes showed characteristic features of peripheral or mixed vascular pattern and grade Ⅱ or grade Ⅲ vascularity, the sensitivity, specificity and accuracy of peripheral or mixed vascular pattern in detection malignant cervical lymph nodes were 76.5% ,93.3% and 84.4% respectively. There were no significant difference between benign and malignant cervical lymph nodes in blood flow velocity and vascular resistance. Conclusion The occult cervical lymph nodes metastases of cancer show affluent peripheral blood flow. PDI has important value in the diagnosis of the occult cervical lymph nodes metastases.
