70岁以上老年患者肠梗阻216例诊治分析

目的　总结我院10年来治疗老年肠梗阻诊的治经验。方法　回顾性分析10年间收治的2 16例老年肠梗阻资料。结果　病因明确198例(91.6% ) ,其中机械性肠梗阻180例(90 .9% ) ,原因不明18例(8.4% ) ;手术治疗10 6例(4 9% ) ,非手术治疗110例(5 1% ) ;总死亡率9.2 %(2 0例) ,手术治疗死亡4例(3 .8% )。结论　老年性肠梗阻病因以粘连性、肿瘤性为常见;高龄和并存病不是手术的禁忌;做好围手术期处理,把握好手术指征,选择合理的治疗方式是保证治疗成功的关键。

Review of the management of intestinal obstruction of 216 over-70 years patients

Objective To reveiew our experience in treatment of senile patients with intestinal obstruction in the recent 10 years.Methods The data of 216 senile patients with intestinal obstruction were analyzed retrospectively.Results The obstructive factors in 198 cases(91.6%) were definite,including mechanical obstruction(180 cases,90.9%),and there were 18 cases(8.4%) with unknown causes.106 cases (49%) underwent surgical treatment and 110 cases(51%) subjected to nonsurgical treatment.Total mortality was 9.2%(20 cases).The operative mortality was 3.8%(4 cases).Conclusion The common factors of senile intestinal obstruction were adhesiveness and tumor.The senium and comorbidities were not the contraindication of surgical treatment.Perioperative management,holding of the operative indications and the selection of reasonable procedures are the key to the successful treatment.
