锁定加压钢板内固定治疗肱骨近端骨折疗效分析

目的：探讨锁定加压钢板（LCP）治疗肱骨近端骨折的疗效。方法：对2004年1月-2009年12月就诊的肱骨近端骨折患者37例进行随访，年龄45～69岁，其中NeerⅡ型骨折试行手法复位失败3例，NeerⅢ型骨折23例，NeerⅣ型骨折11例。分别对术后8周骨折愈合率、术后肩关节功能恢复情况进行分析，肩关节功能评分分别按Neer评分及Constant Murley绝对值标准评分两种评分方法进行。结果：37例患者术后8周均达到临床愈合标准，愈合率100％。肩关节功能Neer评分结果：优25例，良10例，中1例，差1例，优良率94．6％。Constant—Murley绝对值评分结果：优24例，良10例，中2例，差1例，优良率91．9％。结论：应用LCP治疗NeerⅢ，Ⅳ型肱骨近端骨折，骨折愈合及肩关节功能恢复均良好，是临床上比较理想的治疗方法。

Curative effect of locking compression plate in the treatment of the proximal humerus fracture

Objective：To analyze the curative efftects of proximal humerus fracture treated by locking compression plate （LCP）. Methods;Thirty-senven proximal humerus fracture eases （ranged from 45 to 69 years old,average age 55±10 years old） were followed up from January 2004 to November 2009. Three type II （failed by therapy of manipulative reduction）,23 type II and 11 type IV cases which based on neer classification were included in this investigation. Fracture healing rate of 8 weeks and postoperative recovery of shoulder joint were analyzed respectively. Treatment efficacy score of shoulder joint were estimated by constant-murley absolute value score and Neer scoring system respectively. Results：All the 37 cases achieved the clinical fracture healing standard. Postoperative fracture healing rate of 8 weeks was 100%. The fine rate of constant-murley absolute value score and neer scoring system were 94.6% and 91.9% respectively. Conclusion ：The Fracture healing and the function of shoulder joint were all good by the treatment of LCP,suggested that it was an effective method in the treatment of neer type III , IV proximal humerus fracture.
