摘    要:目的 探讨云南普通人群女性阴道微生态异常中细菌性阴道病 (bacterial vaginosis, BV)、滴虫性阴道炎(trichomonas vaginitis, TV) 、外阴阴道假丝酵母菌病(vulvovaginal candidiasis, VVC)、阴道清洁度降低(III~IV)、pH值升高(pH>4.5)与高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)感染的相关性。方法 采用横断面研究设计,研究对象招募于云南省新平县年龄为20~65岁的普通女性人群。纳入者按HR-HPV感染情况分组,组间率比较采用χ2检验,单因素分析采用logistic回归,进而分析上述阴道微生态异常与HR-HPV感染的相关性。结果 入组研究对象HR-HPV阳性率为5.7%(28/491),BV、TV、VVC、清洁度III~IV及pH值>4.5在HR-HPV阳性组中的发生率分别为28.6%(8/28)、10.7%(3/28)、10.7%(3/28)、21.4%(6/28)、78.6%(22/28);在HR-HPV阴性组中发生率分别为16.4%(76/463)、0.6%(3/463)、15.3%(71/463)、7.1%(33/463)和58.5%(271/463)。TV、清洁度III~IV、pH>4.5在HR-HPV阳性组中发生率较高,且差异具有统计学意义(均P<0.05),OR值及95%CI分别为18.400(3.533~95.822)、3.554(1.348~9.371)、2.598(1.034~6.528)。而BV及VVC在两组中分布差异无统计学意义。结论 云南普通人群女性HR-HPV感染率较低;TV、清洁度III~IV、pH>4.5可能与HPV感染存在相关性。

Abstract:Objective To study the correlation between abnormal vaginal micro-ecology such as BV (bacterial vaginosis), TV (trichomonas vaginitis), VVC (vulvovaginal candidiasis), vaginal depuratory degree reduction (III-IV), pH value increase (pH>4.5) and HR-HPV infection among normal women population in Yunnan. Methods This was a cross sectional study. A total of 491 subjects aged from 20 to 65-year-old were recruited in Xinping County of Yunnan Province. According to the HPV DNA test results, we divided the subjects into the positive and negative group, used Chi-square test to compare inter-group rate, and then explored the correlation between abnormal vaginal micro-ecology and HR-HPV infection. Results The positive rate of HPV was 5.7% (28/491). The proportions of patients with BV, TV, VVC, low vaginal depuratory degree and pH value increase in HPV positive and negative group were 28.6% (8/28), 10.7% (3/28), 10.7% (3/28), 21.4% (6/28), 78.6% (22/28) and 16.4% (76/463), 0.6% (3/463), 15.3% (71/463), 7.1% (33/463), 58.5% (271/463), respectively. Significant differences (P<0.05) were found in TV, low vaginal depuratory degree and pH value increase in HPV positive women and the OR (95%CI) were 18.400 (3.533 to 95.822), 3.554 (1.348 to 9.371), 2.598 (1.034 to 6.528), respectively. While BV and VVC had no correlation with HR-HPV infection. Conclusions In this study, the HPV infection rate in Yunnan general population is low. TV, low vaginal depuratory and pH value increase might have a positive correlation with HR-HPV infection.
