高渗氯化钠羟乙基淀粉40注射液对内毒素性急性肺损伤大鼠肿瘤坏死因子-α表达的影响

摘    要:目的:研究高渗氯化钠羟乙基淀粉40注射液(HSH40)对内毒素性急性肺损伤(ALI)大鼠肿瘤坏死因子-α(TNF-α)表达的影响,并探讨其在ALI防治中的作用。方法:建立大鼠急性肺损伤模型。将健康雄性SD大鼠54只,体重250~300g,随机分为3组,每组18只。空白对照组(C组)给予等容量的NS,急性肺损伤组(L组)和HSH40组(H组)均经股静脉注射大肠杆菌脂多糖(LPS)5mg/kg,1min后以5ml/kg输注NS或HSH40于10min内输完。检测各组大鼠血气、肺湿/干重(W/D)比值和肺组织髓过氧化物酶(MPO)活性,采用ELISA法检测创伤后0.5、2、4h血清中TNF-α的表达情况,观察肺组织病理形态学变化并进行病理损伤评分。结果:与C组比较,L组TNF-α在给予LPS后0.5h表达开始升高(P<0.01),2h达峰值(P<0.01),以后逐渐下降,而H组较L组则明显下降(P<0.01);与L组比较,H组动脉血气明显改善(P<0.01),W/D和MPO活性均显著下降(P<0.01),同时肺组织的病理损伤程度也减轻。结论:HSH40能够调节炎性介质TNF-α的产生与释放,从而减轻ALI时组织器官炎症反应的病理损害而起保护作用。

Effect of hypertonic sodium chloride hydroxyethyl starch 40 on TNF-a expression of endotoxin-induced acute lung injury in rats

Abstract:Objective:To study the effect of HSH40 on tumor necrosisfactor-α(TNF-α) of endotoxin-induced acute lung injury in rats and discuss its effect on prevention and treatment of ALI.Methods:Models of ALI in rats were induced by LPS(5ml/kg,iv).54 SD rats were divided into 3 groups by random,each group consisted of 18 rats:①control group received the same volume of NS;②LPS group received NS 5ml/kg infusion 1 min after intravenous LPS 5mg/kg;③HSH40 group received HSH40 5ml/kg infusion 1min after intravenous LPS 5mg/kg.Arterial blood gas,the wet/dry(W/D) of the lung weight,lung myeloperoxiase(MPO) activity were measured.The TNF-α expression in serum was examined by ELISA method after LPS for 0.5、2、4hours.Pathologic changes of lung tissue were observed and graded microscopically.Results:As compared with groupC,the expression of TNF-α in groupL was increased at 0.5h(P0.01),reached the peak at 2h(P 0.01),and then gradually declined after LPS was injected via the femoral vein catheter.But TNF-α in groupH was decreased than groupL(P0.01).As compared with groupL,the arterial blood gas and the lung tissue injury were significantly improved,the W/D and MPO were reduced significantly in group H(P0.01).Conclusion:HSH40 can regulate the level of the TNF-α significantly,which alleviates the inflammation reaction and pathologic lesion of ALI.
