中枢神经系统血管外膜细胞瘤的临床特点及其治疗

目的 探讨中枢神经系统血管外膜细胞瘤的临床特点和诊治方法.方法 总结111例中枢神经系统血管外膜细胞瘤患者的临床表现、影像及病理特点以及手术切除和预后情况.结果 111例均行手术治疗,其中全切除85例,近全切除18例,大部切除8例.术后死亡2例,1例死于消化道出血,另1例死于脑于功能衰竭.术后均行放射治疗.术后77例随访0.5～10年,平均5.5年,51例恢复良好,14例存有神经功能障碍,21例肿瘤复发,12例死亡.结论 中枢神经系统血管外膜细胞瘤生长活跃,复发率高,预后较差.手术辅以放射治疗是主要治疗手段.

Clinical features and treatment of hemangiopericytomas of central nervous system

Objective To study the clinical features,diagnosis and treatment of hemangiopericytomas(HPC) of the central nervous system. Methods The clinical manifestation,image and pathology of 111 cases of HPC treated in our hospital were analyzed retrospectively. Results All 111 cases were operated. 85 cases had the tumor were totally removed,18 cases had subtotal and 8 cases had partially removal of the tumor. Two cases died after operation. All cases received radiotherapy. The average follow-up time was 5.5 year. 51 cases out of 77 cases recovered; 14 cases remained dysfunctional; 21 cases recurred and 12 cases died. Conclusions The prognosis of HPC is poor because it grows actively. The recurrence rate is high. Microneurosurgery and adjuvant radiotherapy are the main treatments.
