超分割放射治疗非小细胞肺癌临床疗效观察

目的：探讨超分割放射治疗非小细胞肺癌治疗效果及毒副作用。方法：对1995年6月--1998年8月将92例NSCLC随机分成超分割放疗(HF)组和常规分割放疗(CF)组。结果：HF组完全缓解率(CR)和总有效率(CR PR)分别为23．9％和89．1％，显著高于CF组的15．2％和60．9％(P＜0．05)。两组1年生存率和局部控制率无显著性差异(P＞0．05)。HF组2、3年生存率和局部控制率分别为52．2％、28．3％和47．8％、37．0％，也显著高于CF组的23．9％、13．0％和26．1％、8．7％(P＜0．05)。HF组急性放射性食管炎发生率较CF组为高(P＜0．05)，放射性气管炎、放射性肺炎和骨髓抑制均无显著性差异(P＞0．05)，全组患者均能耐受治疗，无需特殊处理。结论：超分割放射治疗方案治疗NCSLC明显优于CF方案，毒副作用较轻，患者均可耐受，可提高局部控制率和降低复发率，进一步提高生存率。

ANALYSIS OF HYPERFRACTIONATION RADIOTHERAPY FOR NON-SMALL CELL LUNG CARCINOMA

Objective: To evaluate the effect and acute responses of hyperfractionation radiotherapy for non small cell lung carcinoma (NSCLC) . Methods: From June, 1995 to August, 1998, 92 patients with NSCLC were randomized into two groups: hyperfractionation (HF) group and conventional hyperractionation (CF) group. Results: The short term complete response (CR) and the overall response rate (CR+PR) of HF group were 23.9 % and 89.1 % respectively,those of CF group were 15.2 % and 60.9 % ( P <0.05 ). The 1 year survival rates and local control rates of two groups were not markedly different ( P >0.05). The 2 ,3 year survival rates and local control rates of HF group were significantly improved when compared to CF group , being 52.2 % vs 23.9 % ,28.3 % vs 13.0 %,47.8% vs 26.1 %,37.0 % vs 8.7 %( P <0.05).Acute radiation esophagitis in HF group was significantly higher than that of CF group( P <0.05) and acute radiation pneumonitis,induced tracheitis or the toxicity of marrow occurrence rates were not ( P >0.05).Conclusion:The effect of hyperfractionation radiotherapy for NSCLC was very good,and without obvious side effect.HF can improve tumour control rate,decreasing recurrent rate and increasing survival rate.
