主动脉内气囊造型及控制的仿真研究

摘    要:主动脉内气囊反搏(IABP)在临床中的应用非常广泛,是心血管病治疗的重要手段。我们利用心血管系统的计算机仿真模型,研究了主动脉内气囊的造型及控制参数对反搏效果的影响,并对上述参量进行了一系列优化,结论如下:无论双腔、三腔还是多腔气囊,尾囊的作用最为关键,如能保证其体积适当、直径略小于气囊的其他部分、且最早充气、最晚排气,得到的反搏效果将明显优于单囊反搏的情况。

Abstract:Cardiovascular diseases have been proved to be one of the main causes of death. Intra-aortic balloon pumping (IABP) is the only mechanical cardiac assist device in widespread clinical use. As balloon configuration and control have great influence on IABP effectiveness, to provide the most possible benefits of IABP, we use a non-linear multi-element digital hemodynamic model and simulation method to optimize configuration and control parameters of multi-chambered balloons. We use a model with 11 aortic segments, a heart-lung circulation and lumped venous and peripheral system. (Based on a model developed by Jaron et al.)The results of our simulation experiments show that the greatest benefits can be obtained when: (1)the distal chamber of the balloon is inflated the first and deflated the last, (2)the volume of the distal chamber equals to 35% of the total volume, (3) the radius of the distal chamber is larger than that of the other parts of the balloon and is close to the radius of aorta. Under such circumstances, multi-chambered balloon pumping can provice 15-17% greater Qp than that of same volumed single chamber balloon.
