曲美他嗪与卡维地洛联合治疗风湿性心脏病伴慢性心力衰竭的疗效分析

目的观察曲美他嗪联合卡维地洛治疗风湿性心脏病慢性心力衰竭的临床疗效。方法随机将80例风湿性心脏病慢性心力衰竭患者随机均分为对照组和治疗组,对照组采用常规抗心力衰竭治疗,在对照组的基础上,治疗组加服曲美他嗪和卡维地洛,观察两组的临床疗效及治疗前后的LVEDD、LVESD、LVEF、HR的变化,并比较两组治疗后的血脂情况。结果经过治疗后,对照组的总有效率为67.5%,治疗组的总有效率为92.5%,两组之间的总有效率差异有统计学意义（χ2=8.253,P=0.016〈0.05）;治疗前,两组的LVEF、LVESD、LVEDD、HR、心力衰竭症状评分比较差异无统计学意义（P〉0.05）,经过治疗后,两组的LVEF、LVESD、LVEDD、HR、心力衰竭症状评分均出现不同程度的下降,但治疗组的下降程度更加明显,两组治疗后的LVEF、LVESD、LVEDD、HR、心力衰竭症状评分差异有统计学意义（P〈0.05）;两组治疗后的TC、LDL、HDL、TG差异有统计学意义（P〈0.05）。结论卡维地洛联合曲美他嗪治疗风湿性心脏病慢性心力衰竭的临床疗效较好,能够有效地改善心脏功能,并控制血脂水平,值得在临床更广泛地应用与推广。

Analysis on clinical effect of trimetazidine,combined with carvedilol in treatment of chronic heart failure in rheumatic heart disease

ObjectiveTo observe the clinical effect of trimetazidine combined with carvedilol in treatment of chronic heart failure in rheumatic heart disease.Methods 80 patients with chronic heart failure of rheumatic heart disease were randomly and equally divided into control group and treatment group, control group were gave treatment of regular basis for resistance of heart failure; on basis of the control group, the treatment group was treated with trimetazidine combined with carvedilol, the clinical effects, LVEDD, LVESD, LVEF, HR before and after treatment were observed, and blood lipid after treatment were compared between the two groups.ResultsAfter treatment, total effective rate in the control group was 67.5%, total effective rate in the treatment group was 92.5%, total effective rate between the two groups had significant difference（χ2=8.253,P=0.0160.05）; Before the treatment, LVEF, LVESD,LVEDD, HR, heart failure symptom scores of two groups had no obvious difference（P0.05）, after treatment, LVEF,LVESD, LVEDD, HR, heart failure symptom scores of two groups became varying degrees of decline, but the drop degree of the treatment group was more obvious, LVEF, LVESD, LVEDD, HR, heart failure symptom scores after treatment between the two groups differed significantly（P0.05）; TC, LDL, HDL, TG after treatment between two groups had significant difference（P0.05）.ConclusionThe clinical curative effect of trimetazidine combined with carvedilol in treatment of chronic heart failure in rheumatic heart disease is good; the treatment can improve heart function, and can control lipid levels, it Worthies more widely in clinical application and promotion.
