全胃切除与近端胃切除治疗贲门癌的临床效果比较

目的探讨全胃切除术和近端胃切除术治疗贲门癌后生存期及并发症的差别。方法将245例贲门癌患者分为两组,其中全胃切除术组（total gastrectomy TG组）176例,近端胃切除术组（proximal gastrectomy PG组）69例。结果 TG组患者3、5年生存率分别为44.6%和24.5%,PG组为分别30.2%和17.7%,2组患者的3、5年生存率差异有显著性（P〈0.05）。TG组术后并发症的发生率为6.5%,PG组为29.7%。2组并发症发生率差异有统计学意义（P〈0.05）。结论相对于近端胃切除术,全胃切除术治疗贲门癌可明显降低术后并发症的发生率,提高远期治疗效果。

A comparison study of total gastrectomy versus proximal gastrectomy for the carcinoma of gastric cardia

Objective To investigate the difference of survival and complications between the total gastrectomy （TG） and proximal gastrectomy （PG） in treating the carcinoma of gastric cardia. Methods 245 patients with cardia cancer were divided into two groups, 176 cases in TG group and 69 cases in PG group. Results The 3 and 5 years survival rate in TG group were 44.6% and 24.5% respectively.However, the the 3 and 5 years survival rate in PG group were 30.2% and 17.7% respectively.There was a significant difference （P0.05） in 3 and 5 years survival rate of 2 groups.The incidences of postoperative complications were 6.5% in TG group and 29.7% in PG group respectively.A significant difference （P0.05） was existed in complications of two groups. Conclusion Compared with PG, full stomach resection of cardia cancer treatment can obviously reduce the incidence of postoperative complications, and improve long-term treatment effect.
