冠心病患者心肺功能指标测定的临床意义

摘    要:目的:观察心肺运动试验中的氧摄取动力学、无氧阈及心肺储备功能指标对冠心病的诊断价值。方法:对68例胸痛患者行心肺运动试验,彩色心超测定左室射血分数(LVEF),及冠心病造影,根据冠脉造影结果分成非冠心病组、冠心病组,进行组间二氧化碳呼吸当量、氧摄取动力学及无氧阈、LVEF等的比较。结果:与非冠心病组相比,冠心病组中的VO2peak(20.67±6.01)vs(26.63±4.51)ml/kg/min,P0.001]、VCO2(1.34±0.52)vs(1.72±0.49)l/min,P0.01]、AT(1.21±0.36)vs(1.48±0.32)l/min,P0.01]、O2pulse peak(10.39±3.04)vs(12.33±2.53)ml/beat,P0.01]、VT(1.37±0.43)vs(1.56±0.32)L,P0.05]、VE/VCO2@AT(31.18±4.79)vs(28.88±4.04),P0.05]都有显著性差异。结论:冠心病与非冠心病患者在VO2peak、AT、O2pulse peak等心肺运动的相关指标存在一定的差异。

The Difference of Cardiopulmonary Indexes between Coronary Heart Disease and Non-coronary Heart Disease

Abstract:Objective The purpose of this study was to estimate the utility of kinetics of oxygen uptake(VO2),anaerobic threshold(AT) and cardiopulmonary functional capacity during the exercise test in the assessment of patients with suspectable angina.Methods Treadmill graded exercise tests with gas analysis 、coronary angiography were conducted on sixty-eight patients with angina.According to the result of coronary angiography,68 patients were divided into coronary heart disease group(39 cases) and non-coronary heart disease group(29 cases).The difference of cardiopulmonary functions between two groups was evaluated.Results Compared with non-coronary heart disease group,coronary heart disease group showed statistical significance in VO2 peak(20.67±6.01)vs(26.63±4.51)ml/kg/min,P0.001]、VCO2(1.34±0.52)vs(1.72±0.49)l/min,P0.01]、AT(1.21±0.36)vs(1.48±0.32)l/min,P0.01]、O2 pulse peak(10.39±3.04) vs (12.33±2.53)ml/beat,P0.01]、VT(1.37±0.43) vs(1.56±0.32)L,P0.05]and VE/VCO2@AT(31.18±4.79)vs(28.88±4.04),P0.05].Conclusions VO2 peak、AT and O2 pulse peak have potential value in diagnosing of coronary heart disease.
