贵州黔西南地区少数民族与汉族心房颤动患者ACE基因I/D多态性研究

摘    要:目的:探讨贵州黔西南地区少数民族、汉族心房颤动与血管紧张素转换酶(ACE)基因I/D多态性之间的关系.方法:在贵州黔西南地区选取111例住院房颤患者,分为少数民族组(51例)和汉族组(60例),收集患者一般资料及常规检查结果,采用聚合酶链式反应、基因测序方法检测两组患者血液中ACE基因I/D多态性,分析ACE基因I/D多态性与房颤关系.结果:两组患者一般资料比较,少数民族组房颤发病年龄、并发原发性高血压、左房前后径均小于汉族组,尿酸、左室射血分数高于汉族组,差异具有统计学意义(P<0.05);少数民族组ACE基因I/D的基因型以ID型(43.1%)为主,其次为Ⅱ型(41.2%)和DD型(15.7%),等位基因Ⅰ和D分布频率分别为63%和37%;汉族组基因型以Ⅱ型(46.7%)为主,其次为ID型(28.3%)和DD型(25%),等位基因I和D分布频率分别为61%和39%;两组ACE基因I/D多态位点基因型频率和等位基因频率,两组ACE基因I/D各基因型左房前后径、左室舒张期末内径大小比较,差异均无统计学意义(P>0.05);ACE基因I/D各基因型在男女性别分布上无论是组内还是组间比较,差异均无统计学意义(P>0.05).结论:贵州黔西南少数民族及汉族房颤患者中ACE基因I/D的基因型分别以ID型和Ⅱ型为主,未发现ACE基因I/D多态性的种族差异性,该基因多态性分布不受性别的影响;少数民族房颤发病年龄、并发原发性高血压、左房前后径均小于汉族.

Abstract:Objective: To investigate the relation between angiotensin I converting enzyme (ACE) gene I/D polymorphism and atrial fibrillation in Minority groups and the Han patients in southwest part of Guizhou province. Methods: 111 hospitalized atrial fibrillation patients in southwest part of Guizhou were selected, 51 patients in Minority group and 60 in Han group. General information and routine test results were collected. To investigate the ACE gene I/D polymorphism and its correlation with atrial fi- brillation in the two groups by adopting the methods of polymerase chain reaction, gene sequencing and statistical analysis. Results: (1) General information comparison indicated that onset age was younger, compliced with primary hypertension rates were lower, left atrial diameter (LAD) was shorter, uric acid and left ventricular ejection fraction (LVEF) were higher in minority groups than those in Han group, differences had statistically significance ( P 〈 0.05 ). (2) The genotype distribution of ACE gene I/D in Minority group with atrial fibrillation was mainly typed ID (43.1%), followed by 1/ (41.2%) and DD ( 15.7% ), and the allele frequency of I and D were 63% and 37% respectively; the geno- type distribution of ACE gene I/D in Han group with atrial fibrillation was mainly typed 11 (46.7%), followed by ID (28.3%) and DD (25%), and the allele frequency of I and D were 61% and 39% respectively. Differences in two groups' genotype distribution of ACE gene I/D and allele frequencies, and LAD, LVEDd of patients with different ACE gene I/D genotypes in the two groups were not signifi- cant (P 〉 0. 05 ). (3) There was no statistical significance in ACE gene I/D genotypes comparison on gender distribution either in or between groups (P 〉 0.05 ). Conclusion: The ACE gene I/D geno- types of both Minority group and Han are mainly typed ID and Ⅱ in southwest Guizhou. No racial differences about ACE gene I/D polymorphism was found.
