摘    要:目的检测人类食管癌组织是否有螺杆菌基因,以初步探讨螺杆菌的感染是否与人类食管癌的发生发展相关。方法收集食管癌患者的手术切除标本34例,采用聚合酶链反应(PCR)扩增螺杆菌16SrDNA,阳性者再扩增幽门螺杆菌(Hp)的特异基因glmM。随机选2例阳性标本的PCR产物进行分子克隆、测序、序列分析和近缘细菌遗传进化树比较。对阳性标本进行针对幽门螺杆菌菌体蛋白的免疫组化染色。结果32.3%(11/34)的食管癌组织中发现有螺杆菌16SrRNA基因存在。2例测序及同源性比较和分析显示,食管癌组织中螺杆菌序列与Hp的16SrDNA序列有99%同源性。进化树显示2例食管癌组织中螺杆菌序列与Hp的进化距离最近。所有的阳性标本均扩增出Hp特异基因glmM。免疫组织化学染色显示阳性标本的幽门螺杆菌的蛋白表达主要位于细胞质和间质。结论人类食管癌组织中检测出螺杆菌特异基因和Hp特异性DNA,提示食管癌组织存在螺杆菌慢性感染,可能与人类食管癌的发生有关。

Abstract:Objective To explore if there is infection of Helicobacter species in esophageal carcinoma.Methods Thirty-four surgically resected specimens were randomly collected and studied.Helicobacter species were examined by polymerase chain reaction(PCR) using Helicobacter-specific 16S rDNA primers.Products amplified from two samples were cloned and sequenced and phylogenetic analysis of bacteria was made.A search for other gene that is specific for Helicobacter pylori(H.pylori) was also carried out by PCR.The immunohistochemistry staining for H. pylori was performed.Results Helicobcter-specific 16S rRNA gene was positive in 32.3 %(11/34) samples of esophageal squamous carcinoma in which H.pylori-specific gene glmM in all the samples was positively amplified.Sequence of the cloned 16S rDNA of Helicobacter species from 2 samples were in homology of 99 % with 16S rDNA of H.pylori.H.pylori protein was located in interstitial tissue and the cytoplasm of tumor cells.Conclusion Helicobacteraceae were detected from human esophageal carcinoma tissues,suggesting the infection of Helicobacter species likely H.pylori and an implication of chronic infection with Helicobacter species in human esophageal carcinoma.
