非体外循环冠状动脉旁路移植术的临床应用

目的:探讨非体外循环冠状动脉旁路移植术(OPCAB)的手术经验及临床应用。方法:回顾分析我院自2004年3月-2007年12月采用非体外循环不停跳下进行冠状动脉旁路移植术治疗冠心病37例的临床资料,全组均应用左乳内动脉吻合前降支,2例患者术前及术中应用主动脉内球囊反搏(IABP)临时辅助。结果:全组37例共行冠脉搭桥109支,平均2.94支/人,3例搭桥4支,29例搭桥3支,5例搭桥2支。全组死亡1例,全组无出血和脑部并发症发生。结论:非体外循环冠状动脉旁路移植手术是安全可行的手术方式,可避免体外循环的并发症,手术效果满意。

Clinical Application of Off-pump Coronary Artery Bypass Grafting

Objective:To study the experience of the procedure of coronary artery bypass grafting under nonextracorporeal circulation.Methods:Thirty-seven patients with coronary artery disease undergoing coronary artery bypass grafting under systaltic nonextracorporeal circulation from 1March,2001 to 31December,2007 were retrospectively analysed.The left internal mammary artery was connected to the left anterior descending coronary artery by anastomosis in all the patients.In 2 patients an intra-aortic balloon pump was temporarily inserted as a supplimentary measure before and during operation.Results: In all the 37 patients with coronary artery disease there were 109 stenotic coronary vessels undergoing surgical revascularization with an average 2.94.There were 3 patients with 4 stenotic coronary,5patients with 2 and 29 patients with 3,one patient died.Bleeding and cerebral hemorrhage didn't happen in the surgical procedure.Conclusion:Coronary artery bypass grafting under nonextracorporeal circulation is a safety and feasible operation procedure.The procedure complications such as bleeding and cerebral hemorrhage happen less in nonextracorporeal circulation.Satisfactory effect can be obtained under the precedure.
