MRI动态增强在诊断垂体微腺瘤中的应用

摘    要:目的:探讨MRI动态增强在诊断垂体微腺瘤中的应用价值。方法:回顾性分析49例经实验室、影像学及临床诊断为垂体微腺瘤的病例,分析其在平扫、动态增强及延迟扫描的MR信号改变特点。结果:(1)平扫:1例正常(垂体信号及形态未见异常改变)占2.0%;39例垂体柄偏移,占79.6%;36例垂体内有低信号改变,占73.5%,13例呈等信号,占26.5%;34例垂体局限性向上突起,占69.4%。(2)动态增强47例,垂体瘤早期强化延迟,占95.9%,瘤体intensity—flow曲线峰值在70~100s。(3)延迟扫描35例,垂体肿瘤信号稍低于正常垂体组织,14与正常垂体组织基本一致。结论:动态增强,特别是在增强早期对诊断垂体瘤具有较高准确性,平扫+动态增强可作为影像学诊断垂体微腺瘤的常规检查方法。

Abstract:Objective:Discussing the application of MRI dynanic enhancement in diagnosing pituitary microadenomas. Method:review and analyse 49 cases which are diagnosed as pituitary microadenomas by experiments, images and clinical diagnosis. Analysing their characters of changing MR massages under plain scan, dynanic enhancement and delayed scanning. Results:(1)plain scan: one case is normal(pituitary massage and image are normal which takes 2.0%); pituitary stalk happens in 39 cases, which is 79.6%; weak massage change happens in 36 cases, which is 73.5%. The massage is equal in 13 cases, which is 26.5%; Partly protruding up happens in 69.% of them, 34 cases. (2) 47 cases(95.9%) of dynanic enhancement delay, the top of intensity-flow chart is between 70100s. (3)the massage of 35 cases of application of MRI dynanic enhancement in diagnosing pituitary microadenomas is lower than normal pituitary one. 14 cases are almost the same as normal pituitary one. Conclusion: dynanic enhancement, especially in the early period, has high accuracy in the diagnosis of application of MRI dynanic enhancement in diagnosing pituitary microadenomas. Plain scan and dynanic enhancement can by applied as a common examination of the image science in diagnosing pituitary neoplasms.
