手术和125I粒子近距离照射治疗口面部恶性肿瘤的疗效探讨

摘    要:目的探讨手术联合125I放射性粒子近距离照射治疗口腔颌面部恶性肿瘤的临床效果。方法 56例口腔颌面部恶性肿瘤患者在手术治疗的同时或手术治疗后1周,采用125I放射性粒子组之间植入近距离照射治疗,观察其术后的并发症,及有无复发等情况。结果所有病例随访1~12个月,仅有1例出现皮肤伤口不愈,1例皮肤潮红,余54例患者疗效满意,未见明显出血、感染及颌骨损伤,所有患者未见局部复发及淋巴结转移,生存状况良好,不良反应率为3.6%。结论手术联合125I放射性粒子近距离照射治疗口腔颌面部恶性肿瘤作为一种新的临床治疗途径,并发症少,局部控制率高,近期效果令人满意,有良好的临床应用前景。

Abstract:Objective To explore the clincal effect of operation combined With 1251 seed brachytherapy on oral and maxillofieal malignancy. Methods Totally 56 patients with oral and maxillofical malignancy were treated by tzsI Seeds implanting while in surgery or in the week after surgery. Postoperative complications and the recurrence were observed. Results All patients were followed up for 1 - 2 years. Wounds disunion were shown in 1 patient, skin redness in 1 pa- tient, and the other 54 patients had no discomfort and side effect. The rate of discomfort is 3.6%. All patients had no local recurrence and no lymph node metastasis. Condusions Operation combined with 1z5I seed brachytherapy on oral and maxillofical malignancy is a new clinical approach, with advantages of fewer complications and higher local control rate. The short - term effect of the braehytherapy is satisfactory, and it has good prospects for clinical application.
