IgA和IgG型抗中性粒细胞胞质抗体联合检测诊断韦格纳肉芽肿的价值

【目的】探讨IgA和IgG型抗中性粒细胞胞质抗体（ANCA）联合检测在韦格纳肉芽肿（WG）临床诊断中的应用价值。【方法】对28例临床诊断为wG及172例其他自身免疫性疾病患者血清，应用间接免疫荧光法检测ANCAIgA和IgG型抗体。【结果】wG病患者胞质型ANCA（cANCA）IgG型抗体和cANCAIgA型抗体阳性率最高，均显著高于其他自身免疫性疾病，且差异有显著性（P〈0．05）。联合检测ANCAIgG及IgA亚型抗体阳性率为89．3％（25／28），明显高于单独检测ANCAIgG或IgA亚型阳性率53．6％（15／28）和35．7％（10／28），且差异有显著性（P〈0．05）。【结论lIgA和IgG型抗中性粒细胞胞浆抗体联合检测有助于wG的早期准确诊断。

Application Value of Antineutrophil Cytoplasmic Antibodies (ANCA IgA and IgG) in the Diagnosis of Wegener's Granulomatosis

Objective]To investigate the clinical significance of ANCA IgA and ANCA IgG in the clinical diag‐nosis of Wegener's granulomatosis .[Methods]ANCA IgA ,and ANCA IgG were detected by indirect immunofluo‐rescence in 28 patients with Wegner's Granulomatosis ,in 172 patients with other auto‐immune diseases ,and 100 healthy people .[Results]The positive rates of ANCA IgA and ANCA IgG in patients with Wegener's Granuloma‐tosis were higher than those in the patients of all other groups( P < 0 .05) .In combination of both ANCA IgA and Ig G ,the positive rate was 89 .3% (25/28)‐which was higher than the positive rates of the two detected separately ( P < 0 .05) .The positive rate of of ANCA IgG and ANCA IgA in the Wegner's granulomatosis patients ,detected separately ,was 53 .6% (15/28) and 35 .7% (10/28) ,respectively .[Conclusion]The combined detection of IgA and IgG antineutrophil cytoplasmic antibodies helps early and accurate diagnosis of Wegener's granulomatosis .
