MK-1精密附着体义齿修复Kennedy Ⅱ类牙列缺损对基牙牙周的影响

【目的】探究M K‐1精密附着体义齿修复KennedyⅡ类牙列缺损对基牙牙周的影响。【方法】回顾性分析本院2012年8月至2014年8月收治的KennedyⅡ类牙列缺损且行M K‐1精密附着体义齿修复的35例患者的临床资料，将其作为研究组。另收集同一时期35例行普通卡环义齿修复的KennedyⅡ类牙列缺损患者的临床资料，并将其作为对照组。随访1年，比较两组基牙牙周相关指标变化，美观满意度及舒适度。【结果】修复前两组患者SBI、AL无明显差异（P ＞00．5），修复后均显著改善（P ＜00．5），研究组显著优于对照组（P ＜00．5），组内比较除对照组修复后6个月与修复后1年比较无明显差异外，其余组内比较差异均有统计学意义（ P ＜00．5）；修复后0个月两组患者GCF、GCF‐ALP比较无明显差异（ P ＞00．5），随访期间均呈上升趋势，研究组组内比较差异无统计学意义（P ＞00．5），对照组则显著升高（P ＜00．5），远远高于研究组（P ＜00．5）；研究组美观满意度及舒适度显著高于对照组（ P ＜00．5）。【结论】MK‐1精密附着体义齿修复KennedyⅡ类牙列缺损与普通卡环义齿比较可显著改善基牙牙周状况，提高美观满意度与舒适度，具有良好的应用前景。

Reparative Effects of MK-1 Precision Attachment Dentures on Periodontium of Patients with Ken-nedy Class Ⅱ Dentition Defects

Objective]To explore the reparative effects of MK‐1 precision attachment dentures on periodontium of patients with Kennedy ClassⅡdentition defects .Methods]The clinical data of 35 patients with Kennedy classⅡdentition defects who were treated with MK‐1 precision attachment denture repair in our hospital from August 2012 to August 2014 were retrospectively analyzed and chosen as the study group .In addition ,the clinical data of another 35 patients also with Kennedy classⅡdentition defect but who were treated with ordinary clasp denture repair during the same period were ret‐rospectively analyzed and noted as the control group .Followed up for 1 year ,the changes of the indexes of periodontium , aesthetic satisfaction ,and comfort were observed and compared between the two groups .Results]There were no obvious differences in the SBI (sulcus bleeding index) and AL(attachment loss) levels between the two groups before repair ( P>0 0.5) ,and the SBI and AL levels in both groups significantly improved after repair ( P <0 0.5) .The levels in the study group were significantly better than those in the control group ( P <0 0.5) .Six months and one year after repair ,the SBI and AL levels in the study group got improved further ( P <0 0.5) ,however ,there was no significant change of the SBI and AL levels in the control group in the same period ( P >0 0.5) .There were no significant differences in the GCF(gin‐gival crevicular fluid) and GCF‐ALP (gingival crevicular fluid‐alkaline phosphatase) levels between the two groups 0 months of repair ( P >0 0.5) ,which showed an increasing trend in the control group during the following‐up period ,but the differences in the study group were not statistically significant ( P >0 0.5) .The levels of GCF and GCF‐ALP in the control group significantly increased ,which were much higher than those in the study group ( P <0 0.5) .The aesthetic satisfaction and comfort in the study group were also significantly higher than those in the control group ( P <0 0.5) .Conclusion]MK‐1 precision attachment denture procedure in the repair of Kennedy class Ⅱ dentition defect can signifi‐cantly improve the periodontal status compared with the ordinary clasp denture repair .It can improve the aesthetic satis‐faction and comfort ,and has a good application prospect .
